POSITION: 1st Five Site Coordinator
REPORTS TO: KIDS/1st Five Director
SALARY: As per agency salary schedule FLSA: Non-Exempt
HYBRID WORK: Eligible
JOB SUMMARY: The 1st Five Site Coordinator is responsible for the overall operations of the 1st Five contract through the development and maintenance of relationships with primary care providers or other staff of primary care practices; to ensure the three levels of developmental care through Iowa’s Medicaid Early, Periodic Screening, Diagnosis and Treatment program become standard practice.
ESSENTIAL DUTIES:
- Develop and maintain relationships with primary care providers and other staff in primary care practices.
- Development and maintain relationships with community partners, including other types of health care providers and public health human services, to improve the health and human services care system for children.
- As needed, convene community partners and provide educational services specific to 1st Five, referral pathways, appropriate opportunities to partner with 1st and related early childhood and family support issues.
- Works to identify and work with the Primary Care Physician Champion to move other providers and physicians towards incorporating surveillance and developmental screening into practice workflows.
- Performs regular quality assurance and quality improvement activities, reports, and other Department required submissions.
- Assure appropriate and timely training of staff and on-going professional development.
- Review, monitor, and share data with community partners about 1st Five.
- Attends program meetings, trainings, and interaction with the Department and its contracted partners for program implementation.
- Responsible for the oversight of Developmental Support Specialist staff.
- Available to fill in for the DSS and contact referred clients outside of normal business hours if necessary.
- Maintains confidentiality.
- Follows mandatory reporter procedures.
OTHER DUTIES AND RESPONSIBILITIES:
- Act as information sources concerning program.
- Fill in for other staff members when necessary including the Developmental Support Specialist.
- Input, access and retrieve computer data for programmatic needs.
- Assist with reports and statistics.
- Other duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES:
- Minimum qualifications of Bachelor’s degree or higher in education, health education, social work, counseling, nursing, sociology, family and consumer sciences, health and human development, individual and family studies, early childhood, psychology or other health related field; or a Registered Nurse, Licensed Practical Nurse, or paraprofessional working under the direct supervision of the 1st Five Coordinator.
- Must demonstrate professional development competency within the first six months of employment.
- Training and competence in understanding and responding to risk factors affecting children’s social and emotional competence.
- Demonstrated interpersonal skills and organizational skills.
- Knowledge of community resources and ability to access these resources.
- Excellent written and oral communication skills.
- Ability to work as a team member to accomplish goals for effective program coordination and implementation.
- Ability to travel as required, viable means of transportation, and evidence of insurability.
- Ability to perform medium heavy physical work, exerting up to 50 pounds of force on occasion, up to 20-30 pounds frequently, and up to 5-10 pounds constantly, performing such as activities as stooping, kneeling, crouching, reaching, standing, walking, pushing, pulling, lifting and grasping, including some repetitive motion.
