Access Center Program Coordinator - Social Services Job at Choices CCS
The Access Center Services Program Coordinator would be an employee of Choices Coordinated Care Solutions (Choices) but part of a collaborative initiative from Choices and Our Children Our Future (OCOF), the Allen County Systems of Care entity. The purpose of the Access Center is to expedite access to services and resources for youth and families. It builds a network of onsite and offsite organizations working together. The Program Coordinator will be primarily responsible for triaging all consumers coming into CONNECT Allen County, working with the Access Center Manager to coordinate outreach and marketing efforts, and will provide back-up navigation and assistance to all CONNECT Allen County Staff. The incumbent will support and uphold a commitment to racial and ethnic equity and/or antiracism.
Essential Duties and Responsibilities
- Can articulate the mission, vision and practices of the Access Center, Choices, and the local system of care, Our Children Our Future (OCOF).
- Communicates the needs and expectations of the Access Center to youth and families, community partners, and funders.
- Triages consumers to Service Navigators based on the needs of the consumer.
- Triages phone calls that come into CONNECT Allen County.
- Occasionally provides navigation and referral services to consumers who enter, or call CONNECT Allen County seeking support when all other navigators are unavailable.
- Provides limited crisis intervention services to CONNECT Allen County consumers.
- Provides follow up, support, and coaching to consumers of the CONNECT Allen County.
- Coordinates and provides oversight to the CONNECT Allen County Street Outreach team as a subset of Handing Out Hope, a collaboration of street outreach teams/organizations.
- Provides intake, navigation services, and tracking in HMIS for consumers served through Handing Out Hope Street Outreach.
- Provides presentations, outreach, and marketing for CONNECT Allen County at community events, speaking engagements, and other community opportunities to help grow CONNECT Allen County.
- Works with the Access Center Manager to coordinate and provide tours to community affiliates and partners interested in learning more about CONNECT Allen County.
- Collaborates with CONNECT Allen County and community partners.
- Maintains accurate and detailed documentation.
- Provides reports to the CONNECT Allen County Service Navigators and Coordinator.
- Adheres to the policies and protocols established by Choices Coordinated Care Solutions, Inc.
- Incorporates the guiding principles and values of Choices Coordinated Care Solutions, Inc., CONNECT Allen County, and OCOF Allen County Systems of Care into their interactions with consumers, fellow team members, fellow staff, and system partners.
- Communicates effectively with CONNECT Allen County, Choices Coordinated Care Solutions, Inc. staff, The Lutheran Foundation and OCOF leadership.
- Will provide excellent customer service to consumers and partners at CONNECT Allen County and in the community.
- Ensures collection of all necessary service data.
- Oversees CONNECT Allen County emergency pantry, including organization, data collection, distribution, and collaboration with all donation partners.
- Participates in scheduled meetings to review performance and quality indicators and makes recommended adjustments to improve the operation of CONNECT Allen County.
- Willingly completes other duties as assigned to advance the mission of CONNECT Allen County.
Qualifications
- Bachelor’s degree in social work or related human services field preferred.
- Strong communication and writing skills. Bi-lingual skills (Spanish or Burmese) a plus.
- Familiarity with Systems of Care values and principles.
- Highly organized, and effectively manages multiple priorities simultaneously.
- Professionally articulate in communicating (oral and written) service expectations, strengths and needs of clients and outcomes.
- Demonstrated professionalism working in teams with internal colleagues and external audiences in both person and through various electronic media.
- Enthusiastic about advancing system change within the community.
- Valid driver’s license.
- Operational smart phone.
