To ensure the overall safety, security and well-being of elderly clients of the Adult Day Service Center (ADSC). The Activity Leader/Client Services Coordinator will lead activities that engage and stimulate older adults afflicted with memory loss. This role will also assist clients with activities of daily living, including feeding, showering and basic custodial care.

PRIMARY RESPONSIBILITIES

A. Assists with clients safe arrival and departure to the ADSC including signing them in and out on daily roster.

B. Prepares, implements and leads program activities as assigned daily.

C. Escorts and assists clients with personal custodial needs, including showering as needed.

D. Assists with meal and snack preparation and service; provides clients with needed assistance; monitors for general appetite and safety and provides clients with adequate fluids to maintain hydration.

E. Monitors, reports and documents clients meal intake, restroom schedule, changes in health, behavior, and hygiene and program participation.

F. Reports to Supervisor any change in clients health or behavior based on observations and activity project selections.

G. Communicates with family members as appropriate.

H. Uses agency property and supplies in a responsible manner.

I. Assists in maintaining the cleanliness of the facility.

ESSENTIAL JOB FUNCTIONS

1. Ability to assist with clients safe arrival and departure to the ADSC.

2. Ability to prepare, implement and lead program activities as assigned daily.

3. Capable to assist with meal and snack preparation and service; provides clients with any needed assistance; monitors for general appetite and safety and provides clients with adequate fluids to maintain hydration.

4. Ability to position oneself to safely maneuver wheelchair-bound clients (on upward and downward slopes), and/or those using assistive walking devices, involving pushing and controlling the device.

5. Ability to transfer wheelchair-bound clients into and out of their chairs and into and out of vehicles, requiring lifting up to 35 pounds unassisted*

OTHER RESPONSIBILITIES

Due to changing program demands, funding source expectations and/or changes in agency policy, as assigned, all staff may be expected to perform other duties.

QUALIFICATIONS

A. Educational Requirements - Must be at least 18 years of age, with high school diploma or GED (some college preferred).

B. Certified Nurses Assistant Certificate (CNA) strongly preferred.

C. Experience

1. A minimum of one (1) year experience working with older adults with dementia, Alzheimers disease, disabled or other at-risk populations.

D. Skills

1. Energetic, enthusiasm, kindness, creativity, flexibility, punctuality and a commitment to teamwork.

2. Ability to motivate clients to participate in activities of daily living.

3. Ability to communicate clearly and anticipate needs of clients.

4. Basic understanding of problems associated with the impaired older adult preferred including memory loss, physical needs assisting with transfers and dealing with difficult behaviors.

5. Sensitive to the needs of each individual.

6 Leadership skills to implement therapeutic activity program.

7. Good communication/ relationship skills & good judgment.

8. Ability to independently plan, prepare and carry out assignments.

OTHER

1. Must obtain upon hire and maintain a CPR and First Aid card.

2. Must complete Health Screening Report within 3 days of hire (proof of TB clearance is required must be kept current every 4 years as required by the County of Los Angeles).

3. Fingerprint background clearance required.

WORK ENVIRONMENT

  • Assistive mobility equipment is not available onsite. Due to the physical nature of the work involved, the job duties must be performed safely so as not create to overexertion injuries. This position requires transferring and maneuvering individuals. Must be able to maneuver up to 35 pounds.
