WMEC offers a hybrid schedule of 2 days remote and 3 days in the office, after the training period is complete.
POSITION SUMMARY:
The Case Worker provides services as a member of a nursing and social work team that facilitates the screening of new foster homes and program participants. He/she assists in matching and placement services, and maintains services to participants as needed.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Works within the established program policies, procedures, and time lines to provide professional social work/case management services.
2. Provides psychosocial evaluation of referred individuals and potential host families.
3. Develops participant-interdisciplinary care plans based on social work practice with a foster care nurse and the program director.
4. Participates in the assessment of potential host families, including home evaluation and inspections.
5. Conducts a process to match potential participants and host families.
6. Provides casework/case management services to ongoing caseload. This is to include follow up services, discharge planning and crisis intervention.
7. Prepares and maintains necessary, mandated client files, case reports and forms generally on the computer. Maintains and provides thorough and accurate documentation.
8. Participates in and initiates outreach programs to potential participants and caregivers in the Adult Foster Care Program.
9. Attends professional meetings, seminars, conferences and courses. Shares information received with agency staff. Participates in and/or presents agency sponsored training sessions.
10. Appropriately volunteers to assist others when time and responsibilities permit.
11. Assumes special projects within the scope of the responsibilities of this position as assigned by the Adult Foster Care Program Director.
REQUIRED KNOWLEDGE SKILLS AND ABILITIES:
- Possesses the ability and willingness to support the mission statement, goals, policies and quality standards of WestMass ElderCare; and has the ability to interpret, understand and adhere to the regulations, standards, and procedures as set forth by the Agency and the Executive Office of Health and Human Services.
· Possesses a Bachelor’s Degree in social work and previous experience in working with physically and mentally disabled adults and elderly adults in a community setting. An equivalent degree and two years clinical experience in the care of elderly or disabled persons experience may be substituted for the educational requirement.
· Must function within the Professional Social Work/Case management Code of Ethics, with respect for the individual participant’s rights.
· Must be able to handle confidential data and information discreetly.
· Must be able to work effectively in a changing environment.
· Possesses knowledge of federal and state laws and regulations in regard to funded services, or has the willingness and ability to learn such regulations.
· Has demonstrated psychosocial assessment skills.
· Possesses the ability to approach people in a manner, which creates harmony and promotes cooperation.
· Possesses excellent oral and written communication skills including presentation skills.
· Must be attentive to details and capable of accurate work.
· Possesses the ability and willingness to work independently and/or with others.
· Possesses the ability to make appropriate decisions.
· Possesses initiative, which can be directed toward the growth of the Adult Foster Care Program.
· Possesses good listening skills.
· Possesses the ability to effectively cope with a variety of personalities and other stress provoking factors associated with a broad base of people and time constraints.
· Must provide personal and reliable transportation.
· Ability to conduct in-person assessments with consumers
· Ability to conduct telephonic and/or video assessments with consumers using devices provided by WMEC
· Bilingual Spanish Preferred
