Here you have a unique opportunity to improve the lives of people with developmental disabilities who live in their own homes or group homes in the community where you assist these individuals with mobility, communication, bodily function, hygienic, and nutritional needs appropriate to the individuals needs and lifestyle.

You get the opportunity to build authentic relationships with the individuals you support.

You educate, train and coach direct support staff to follow each individual’s health care plan.

You work with other community healthcare providers to care for individuals of all ages with intellectual and developmental disabilities

Provide phone triage, including after hours on-call rotation.

MINIMUM JOB REQUIREMENTS

  • Current LPN Certification
  • A valid driver’s license with no more than 5 points on driving record
  • Age requirements: 18. (If 18-20 years of age, must have NO points on driving record).

JOB QUALIFICATIONS

  • Experience supporting individuals who have developmental disabilities and their families.
  • Previous nursing experience
  • Interest in supporting individuals with disabilities.
  • Demonstrate reliability regarding basic work habits such as attendance and punctuality.
  • The ability to work as a team member.

PHYSICAL DEMANDS/WORKING CONDITIONS

  • Ability to regularly perform physical activities such as lifting (50-100 pounds), carrying, pushing, pulling, bending, transferring, stooping, and walking when needed.

WORK SCHEDULE DEMANDS / WORK LOCATION REQUIREMENTS / TRAVEL DEMANDS

  • Local travel required.
  • Primarily day shift M-F; occasional weekend/evening hours; on-call rotation
