This individual works with physicians, peers, community colleagues, and others to orchestrate care across the health care continuum, to identify opportunities to continually improve patient care and services, to improve population health, and to achieve collaborative practices that exemplify Main Line Health System’s commitment to patient centered care and community engagement as well as improving patient and family health by addressing social determinants of health that create barriers to care.

On-going feedback and career development are provided by your Manager and Supervising Physicians through the use of performance appraisals and 1:1 communication to further develop yourself professionally.

Position: Ambulatory Care Social Worker
Shift: Full time – 40 hours a week – Monday-Friday, no weekends or holidays!

Experience:
At least 5 years of recent experience in a physician’s office, clinic, hospital, or emergency services (EMS) setting, agencies, or other equivalent experience. Leadership experience a plus.

Education:
Master’s Degree in Social Work from CSWE (Council on Social Work Education) accredited program required.
