Assisted Living Program Case Manager
Seeking an Assisted Living Program Case Manager responsible for day-to-day management. Must be a Licensed NYS registered nurse. Prior experience is preferred.
Will coordinate referrals, pre-admission and admission activities. Conduct facility orientation for resident and family members. Coordinates communications with physicians and other health care providers. Conducts assessments and ensures individualized treatment plans are in place and updated as needed. Supervises ALP staff and participates in on-call rotation.
