BILINGUAL CASE MANAGER/ Family Forward
At Signal Centers we are Building Futures and Changing Lives! Family Forward supports families to have healthy pregnancies and successful early childhood development through resource linkage, education, and community-building. Family Forward meet with clients bi-weekly in their homes to provide intensive case management and connect families to needed resources promoting health, wellness, safety, and stability. Job responsibilities include:
- Carry and maintain caseload of 25 families
- Participate in outreach events to recruit new families
- Data entry including case notes, assessments, and other tracking and reporting requirements
- Participate in parent workshops and other community building events
- Assist with the preparation of quarterly family events
- Present the program to partners, funders and other professionals within the community
This is a grant funded position that work with inner city families in the Chattanooga area. Occasional evening or weekend work may be required.
- Bachelor’s degree in Social Work, or a closely related field
- Must be Bilingual in Spanish and English
- Previous experience working with inner cities families in a case management model
- Experience conducting home visits and working with new mothers is highly desired, but not required.
- Must have a valid Driver's License and reliable transportation with valid insurance per TN state requirements.

Pay: $37,000.00 - $39,000.00 per year
