Job Summary:* Provides information regarding available benefits, conducts benefit screening and provides application assistance to low income residents.

Duties & Responsibilities:*

  • Performs and conducts one-to-one and group presentations and education sessions to diverse population.
  • Conducts public benefits screening.
  • Assists participants in completing the benefits enrollment process.
  • Documents outreach contacts and collects/records demographic data on contacts on appropriate tracking forms.
  • Attends all scheduled training, supervision, and agency meetings.
  • Maintains monthly reporting and data entry as required by the contract, including achievement of productivity goals.
  • Assures security of confidential information and materials.
  • Networks and develops relationships with various community service agencies,.
  • Accepts direction and reports to team leader, or coordinator, or manager for direct supervision.
  • Other duties as assigned.

Qualifications:*
Education: High school diploma or GED with 1-2 years experience in human services and/or applicable life experience. Demonstrated experience with in-home, low income, family-based services and/or commensurate experience may qualify. Knowledge: Knowledge of community resources. Abilities: Demonstrated ability to be culturally sensitive and respect diversity. Ability to provide support, structure, empathy, sound judgement, insight into human behavior and family relations. Skills: Effective communication skills. Must be a team player and be able to work independently. Computer literate as appropriate. Bilingual in Spanish preferred.
Other Requirements: Must have a valid California driver’s license, insurance and a reliable vehicle and be willing to use it on the job.

Physical Demands:*

  • Sitting for long periods of time in the homes of client families, in the office or at required meetings.
  • Traveling in car to agency and community meetings when neccesary.
  • Frequent note taking and recording in case files by hand.

Work Environment:*

  • Office space is shared.
  • The noise level fluctuates from quiet to noisy.

Condition of Employment*
In accordance with the recent California Department of Public Health Orders, depending on work location and subcontractor requirements, employees must adhere to the below requirements:

  • Provide a copy of your COVID-19 Vaccination OR undergo weekly COVID-19 testing and observe all other infection control requirements, including face coverings.
  • Obtain either the first dose of a one dose regime or the second dose of a two-dose regime of a COVID-19 vaccine by September 30, 2021* in accordance with CDPH order dated August 5, 2021.
  • There may be exemptions to the vaccination mandate.
