Home Again is seeking a compassionate, responsible, dedicated, knowledgeable leader! The Care Coordinator position serves as a connector for all positions within the agency, family members, and clients we serve! The Care Coordinator position primary functions are:
- Prompt and Consistent communication with clients and team members
- Hiring
- Educating/ Training new hires, current team members
- Scheduling
- Documentation
- Client Assessments
- Coaching and counselling as needed with team members
- Client Care
Must be compassionate, reliable and exemplify excellent communication skills!
Job Type: Full-time
Pay: $27,800.00 - $35,000.00 per year
