Care Coordinator Job at Sentara Healthcare
Sentara Home Care and Hospice is currently hiring a full time Care Transition Coordinator based out of Halifax, VA.
The Care Transition Coordinator is responsible for the identification and assessment of home care and hospice needs of patient population and coordination of these services to ensure a smooth transition to the next site of care.
Requirements:
- 1 year of home care or hospice experience preferred
- Discharge planning or case management experience required.
- Licensed RN, LPN, RT or BSW required.
