Care Coordinator General Description

We are looking for qualified Care Coordinators that are committed to helping members improve their functioning, build self-sufficiency, and meet treatment goals through the following responsibilities:

• Engage, educate, provide emotional support, and ensure member’s psychiatric, physical, and behavioral healthcare needs are met.
• Provide services and coordination based on each member’s needs/requests.
• Coordinate member participation, and compliance, with psychiatric and/or primary health care services.
• Collaborate and coordinate with the member’s support system and treatment team.
• Support the member in accessing resources to advance their service plan goals.
• Observe members, listen to concerns, and document appropriately in the Electronic Health Record.
• Work collaboratively with the member’s integrated healthcare team to assist with improving and maintaining quality care.

• Provide evidence-based interventions or work with other mental health providers when such treatment is indicated.
• Have strong time management skills, knowledge about community resources, and experience working with children, young adults, or adults with a variety of mental health concerns.

• Document and/or report any areas of concerns related to the member’s behaviors/interactions.
• Monitor the health and safety of members and report any concerns to the appropriate staff/leadership.
• Maintain and update member records, including assessments and treatment plans.
• Develop member treatment plans that are time specific and action oriented.
• Complete timely progress notes using covered service billing codes.
• Complete all training required by licensing, agency, or funding sources.
• Monitor and evaluate the effectiveness of each member’s treatment plan.

General Qualifications

• Bachelor’s degree in behavioral health related field; or
• Associate’s degree and 2 years behavioral health experience*
• * Experience maybe considered in lieu of education
• Proof of COVID-19 vaccination required
• Minimum 21 years of age.
• Arizona Level One Fingerprint Clearance Card.
