Care Coordinator II

I. POSITION SUMMARY

The Care Coordinator will provide efficient, comprehensive, and effective care management services to older adult and disabled residents which include a comprehensive assessment of the clients, development of a written care plan with measurable outcomes, arrangement and coordination of services, and client follow-up to monitor the effectiveness of the services arranged and assess the need for additional services.

II. PRIMARY RESPONSIBILITIES

• Conduct client intakes and in-home assessments using program specific forms to identify client needs and confirm eligibility
• Field travel to client’s homes and other sites, as needed, up to 50% of the times.
• Care management responsibilities including the following:
• Conduct a comprehensive in-home assessment and write a biopsychosocial assessment; Complete paper documentation for all progress notes.
• Develop a written Care Plan with measurable outcomes to address and implement an agreed upon action plan for client needs.
• Identify appropriate resources and mobilize them to address client needs and resolve problems.
• Arrange and coordinate services on behalf of client.
• Maintain contact with client to monitor the effectiveness of the service arranged for client and assess the need for additional services.
• Write accurate, thorough, and specific client assessments, letters, emails, and other communications. Complete timely, accurate, and thorough documentation for client files.
• Prepare for and participate in weekly Supervision meetings with Manager and other staff.

III. ESSENTIAL JOB FUNCTIONS

• Ability to perform biopsychosocial assessments of clients using appropriate techniques.
• Ability to document client information and maintain client boundaries and confidentiality.
• Ability to visit clients’ homes with use of personal vehicle or other means of personal transportation.

IV. POSITION QUALIFICATIONS

• Educational Requirements
• Master’s degree in Social Work, Gerontology, Human Services or related field required.
• Licensed Clinical Social Worker (LCSW)
• Minimum three years of experience working with older adults.
• Fluency in English (bilingual in Spanish strongly preferred).Previous experience working with an ethnically and economically diverse population.
• Previous care management experience strongly preferred.
• Experience with medical insurance billing.

Other Requirements

• Must have reliable transportation, current driver’s license, clean driving record, and proof of auto insurance.
• Travel is required within Los Angeles County.
• Must pass a criminal background check.
• Medical billing experience

WORK ENVIRONMENT

The Employee may be expected to enter premises during field visits that may be environmentally challenging: including, but not limited to, environments with cigarette smoke, bed bugs, dust, and other allergens.

This position is based in the corporate office of WISE & Healthy Aging located at 1527 4th Street, Santa Monica, CA 90401.

Pay: From $70,000.00 per year
