MIND & MOBILITY Home Care seeking a friendly, organized, and experienced person to join our West Palm office of operations as our Care Coordinator for our Home Health Care Agency. The ideal candidate must have a professional demeanor, be a self-starter, a problem solver, be able to think outside of the box, have the ability to multitask, and the ability to handle stressful situations.

Some of the responsibilities for this position are:

• Work collaboratively with key team members to understand retention needs and the existing caregiver pool.
• Be able to communicate and build relationships with new prospective employees (mostly certified nurse assistants and home health aides)
• Be able to make office visits to educate prospective referral sources.
• Conduct new caregiver staff orientations - as needed.
• Organize and maintain file systems in compliance with policies and regulations.
• Perform general clerical duties – correspondence, copying, filing, and distribution.
• Assisting with intakes of potential patients and the follow up associated with these potential patients - as needed.
• Responsible for overall employee retention
• Assist with scheduling as needed in staffing cases according to agency guidelines and client needs, being proactive in meeting the needs of both caregiver and client.
• Maintain positive relationships with all clients and referral sources.
• Communicate and refer appropriate matters to direct supervisor for direction.

Office hours are Monday - Friday 8am to 5pm, on call rotation weekends and after-hours.
