Crossnore Communities for Children is seeking Care Manager Extenders for all 3 campus locations (Crossnore, Hendersonville, and Winston-Salem, NC). The Tailored Care Management (TCM) service model provides members access to a multidisciplinary care team dedicated to delivering integrated, whole-person care management addressing a member’s physical health, behavioral health, intellectual/developmental disability (I/DD), traumatic brain injury (TBI), long-term services and supports (LTSS), and/or pharmacy needs, in addition to unmet health-related resource needs. Duties include facilitating referrals, data entry, outreach, and service coordination.

Care Manager Extenders must maintain a flexible workweek, be willing to schedule member meetings during evenings or weekends as needed, and be available to provide after-hours on-call support to staff and member families.

Status: Full Time
Hiring Range: $38,000 - $42,500 annually

FULL JOB DESCRIPTION

JOB TITLE: Care Manager Extender

REPORTS TO: Care Manager Supervisor

SUMMARY : The essential purpose of this position is to support the delivery of Tailored Plan Care Management (TCM) to children with behavioral health needs and their families. Supports the Care Manager by facilitating referrals, data entry, outreach, and service coordination. Helps provide a whole-person approach to ensure that the child’s behavioral health, physical health, and support system needs are met.

STATUS: Full Time, Exempt

PAY SCALE: Hiring Range: $38,000 to $42,500 annually

QUALIFICATIONS:

Education/Licensure
  • A high school diploma or equivalent
  • Peer support specialist certification, preferred

Experience
  • At least two years of direct experience providing care for and navigating the NC Medicaid delivery system on behalf of an individual with a behavioral health condition (as a parent/guardian/professional parent)
OR
  • Have two years of paid experience performing the types of functions described in the job description, with at least one year of paid experience working directly with the target population of the program
OR
  • Personal lived experience with a behavioral health condition and licensed as a Certified Peer Support Specialist (foster care lived experience, preferred)

Skills/Competencies
  • General knowledge of the state child welfare and mental health systems required.
  • Excellent interpersonal, communication and organizational skills; including superior customer service skills..
  • Ability to quickly establish and maintain effective working relationships with coworkers, youth, families, referral agencies, and community collaborators.
  • Demonstrated proficiency in the use of a variety of computer software programs (Microsoft and Google products required).
  • Ability to accurately process, file, track and communicate detailed information. Exceptional attention to detail is required.
  • Demonstrated passion for serving youth and families, and the youth-serving workforce.
  • Ability to exercise compassion and sensitivity, and to maintain confidentiality of client and organizational information.
  • Excellent time management skills, with ability to perform multiple tasks simultaneously and efficiently, and flexibility to adapt quickly to changes.
  • Demonstrated ability to operate well in crisis situations and knowledge of cultural issues that can affect care.
  • Ability to work on diverse teams and must be flexible and willing to work with youth of all ages, races, nationalities, sexual orientations, gender identities, disabilities, and backgrounds.
  • Ability to laugh, be creative and have willingness to make and learn from mistakes.
  • Valid Driver’s License, and must be willing to travel to various job related locations.
  • Must be willing to work a flexible schedule, providing 24 hour coverage to member caseload.

JOB DUTIES & RESPONSIBILITIES
  • Assist the Care Manager with a caseload of anywhere from 30 (high acuity), to 129 (lower needs) members, ages 3 to 21.
  • Gather information for Care Manager assessments, to include a member’s health care needs, functional needs, accessibility needs, strengths and supports, medication history, trauma history, cultural considerations, risk factors, employment/community involvement, goals, and other characteristics that will inform the ongoing care plan or ISP and treatment.
  • Perform general outreach, engagement, and follow-up with members.
  • Coordinate services/appointments (e.g., appointment/wellness reminders, arranging transportation).
  • Engage in health promotion activities (as defined in the Tailored Care Management Provider Manual) and knowledge sharing.
  • Participates in a multidisciplinary care team for each member under Tailored Care Management. Depending on the member’s needs, members of a care team may include caretakers, peer support specialists, medical providers, nutritionists, pharmacists, and other specialists. Shares information with the care manager and other members of the care team on the member’s circumstances.
  • Provide and track referrals and provide information and assistance in obtaining and maintaining community-based resources and social support services.
  • Participate in case conferences.
  • Support the care manager in assessing and addressing unmet health-related resource needs.
  • Assists Care Manager with coverage for services, consultation or referral, and treatment for emergency medical conditions, including behavioral health crisis, 24 hours per day, seven days per week.
  • Build and maintain strong relationships with coworkers, community partners, referral agencies, providers, members.
  • Other duties as assigned.
DAYS/HOURS OF POSITION and WORK CONDITIONS:

Requires working flexible hours, including evenings, weekends and holidays. In collaboration with the Care Manager, must be available to members on caseload 24 hours per day, 7 days a week. Work involves travel to locations that range from human services agencies, clinics, hospitals, offices, and locations for field trips. May involve contact with members and/or family members who may be hostile, resistant, and violent.

Position may require lifting to 20 pounds at a time with frequent lifting or carrying of objects weighing up to 10 pounds. Frequent walking, standing, and sitting. Frequent reaching and/or grasping using hands and/or arms. May be required to ascend/descend stairs. Frequent use of hands requiring dexterity in using the telephone, computer keyboard, or other objects. Visual ability including distance, peripheral and depth perception. Ability to drive. Regularly required to talk and hear.
