STRIDE Community Health Center is a 501(c)(3) Federally Qualified Health Center (FQHC) with a 30-year history of providing primary care and other health-related services in a community-based setting. Established in 1989 in response to the primary health care needs of the medically indigent population in suburban Denver, STRIDE Community Health Center has the largest population base of any FQHC in Colorado and provides primary care, dental, pharmacy, behavioral health, health education, care coordination, patient navigation, pharmacy, and outreach and enrollment services in accordance with its mission.
Summary
The Care Navigator Supervisor will be responsible for the daily operation of Care Navigation and Health Education activities to include supervising and coaching the care navigators and transitions of care staff. In addition, the Care Navigator Supervisor will provide pertinent and preventive health education, in-depth assessment, intervention, coaching, advanced care planning and support to STRIDE patients and their responsible parties to proactively manage their care. This position will also collaborate on the implementation of new programs, and compliance with program and grant guidelines. The Care Navigator Supervisor will assist the Care Navigator with mentoring, training and oversight of the daily performance of staff duties.
Essential Duties and Responsibilities
- Mentor, train, audit, coach and oversee day-to-day operation of the Care Navigation team, specific to health education and care navigation competencies.
- Performs all expected roles and tasks of Care Navigator.
- Oversee the process and tracking of referrals including but not limited to cancer screening referrals. Ensure Care Navigators are completing all steps of the referral process.
- Assist with the development and supervision of Care Navigation programs, projects, workflow, standard operating procedures and reports.
- Conduct individual supervision with each team member to discuss the staff’s performance review, goals and program audits.
- Conduct shadowing and observation as needed, to ensure expectations of the role are being fulfilled.
- Oversees the implementation, monitoring and evaluation of grant programs and ensures effective due diligence and compliance for grants as needed and assigned.
- Coordinates clinic coverage and support in the absence of Care Navigation staff.
- Collaborate with other supervisors and managers to improve processes and workflow as needed.
- Assist with process improvement strategies to enhance the effectiveness and awareness of Care Navigation activities.
- Establish and maintain effective interpersonal relationships with staff at all levels, providers, other departments or programs.
- Prepares detailed and concise reporting on program accomplishments and key performance indicators.
- Build team engagement through organizing team meetings and coordinating trainings, inside and outside the organization, per training guidelines and accessibility.
- Works with Community of Care leadership team to interview, hire and train new employees as needed and conduct employee performance evaluations.
- Work closely and effectively as a liaison between management and the team. Ensure that all issues are brought to manager’s attention in a timely manner.

Education and Experience
- Bachelor’s degree required or in progress. Case Manager experience may be substituted on a year-for-year basis for bachelor’s degree. Supervisory/leadership experience preferred. CHES preferred.
- Previous experience working in social work, behavioral health or community health required. Background in Care Management, health education, or community resources required.
