Avenues to Independence has been providing individual supports and services to adults with developmental disabilities for over 60 years. As a 501C3 charitable organization, employment at Avenues meets the requirements for PSLF participation.
Avenues currently has Caregiver (Direct Support Person) positions available in Park Ridge and Des Plaines.
Responsibilities include:
- Assist in the daily support of persons with intellectual and other developmental disabilities in their home
- Carry out the requirements and participate in the development of each individuals Individual Support Plan
- Assist persons served with daily living skills
- Promote the health and well being of the clients we serve
- Must be qualified to administer medication under Illinois Administrative Rule 116
- Participate in individual and group support sessions
- Participate in agency and family meetings
- Provide crisis intervention when necessary
- Supervise individuals in all of their activities on and off the facility premises
- Provides information to parents and other interested parties while maintaining Avenues standards of confidentiality
- Ensure safe living conditions for all persons served
Minimum requirements for these positions: * Proof of High School Diploma or GED
- Proof of motor vehicle insurance as well as good driving record
- Some experience in social services field preferred but not required
- Ability to meet the established Illinois Department of Human Services DSP training
- Must pass required criminal background checks
- Must be qualified to administer medication under Illinois Administrative Rule 116
