Enable, Inc's adult supported living program designed to provide case management to individuals with disabilities allowing them to live independently in the community. The program is funded through Massachusetts Rehabilitation Commission, Department of Developmental Services, and MassHealth.

The Case Coordinator acts as a direct resource to meet the consumer’s needs. Services are provided in the home, face to face (other than PCA surrogacy). This position requires someone who is highly ethical, self-motivated and detail oriented.

Responsibilities and Duties

  • Provides direct case management services to assigned 6-10 consumers in the geographic area of the program; covers other cases, as requested.
  • Assists consumers to lead, enhance and enrich their lives in the manner of their choosing; participates in the execution of a service plan to support the goals and wishes of the consumer. Makes measurable progress towards these goals and records progress as assigned.
  • Makes weekly home visits to each consumer
  • Provides Personal Care Assistant program surrogacy and management including recruiting, screening, hiring, managing, scheduling and terminating PCAs as necessary.
  • Provides housing assistance in the form of housing searches for subsided, accessible housing. Provides housing advocacy. Understands and apply HUD and state subsidy laws and regulations to advocacy work.
  • Provides education and support for financial management including assistance paying monthly bills, budgeting and understanding benefits. Must have working knowledge of state and federal assistance programs and knowledge of the Medicaid/Medicare systems.
  • Provides assistance with household management including assistance with planning and executing shopping and housekeeping. Provides assistance in planning for emergency situations and preparedness.
  • Assists with personal health care; ensures that consumer doctor’s appointments are attended including attending those appointments with consumers. Assists with understanding medical instructions, monitoring medications and connecting with other sources for medical assistance.
  • Provides coordination of equipment needs; must have a working knowledge of adaptive equipment available and the process to obtain this equipment.
  • Assists with education around transportation including scheduling transportation for consumers as needed and coordinating transportation.
  • Assists with social/educational needs by connecting consumer to various resources.
  • Carries a 24 hour emergency cell phone as scheduled; coordinates emergency service management with other staff as needed.

Qualifications

  • Minimum of BA/BS in rehabilitation counseling, social work, psychology, physical/occupational/speech therapy, nursing, special education, or related field required.
  • Familiarity with population served or related population.
  • Supported living experience or experience in residential services or rehabilitation services.
  • Familiarity with ethical and other considerations in the provision of services in the homes of individuals.
  • Computer competency, particularly with MS Word, Excel and Outlook.
  • Ability to work independently and as part of a team.
  • Able to travel independently throughout MassachusettsAll Staff must report suspected incidents of consumer abuse of any nature (physical, sexual or emotional) or neglect to the Disabled Persons Protection Commission immediately.

This position requires driving to and from client or program sites during the normal course of work but does not allow the transportation of clients in a personal vehicle.
