Options for Community Living, Inc. is looking for a Case Management Assistant to join our Home & Community Based Services (HCBS) team! An ideal candidate should meet the following requirements:
- Must be able to read and comprehend program materials (8th Grade Reading Level or higher).
- Must have adequate interpersonal skills in order to motivate and assist others.
- Must be able to communicate knowledgably about the local services and the systems of care.
- Should be reflective of the population being served with one year of active participation in mental health self-help activities, peer support or peer advocacy program or recipient run organizations or similar programs.
- Must hold a valid NYCPS, NYCPS-P, CRPA or CRPA-P.
- Valid driver’s license, clean driving record and access to a vehicle is required.

Schedule:
Monday, Wednesday, and Thursday: 17.5 hours - 21 hours per week
Responsibilities:
The part-time Case Management Assistant will coach and encourage tenants in self-advocacy and self-management. Provide frequent contact and monitoring of progress and accompany clients on clinical and supportive service appointments.
- Provide direct peer support services with program participants
- Document services through a person-centered, collaborative process
- Provide linkages to community resources
- Provide encouragement and support in community settings
- Promote ongoing recovery through empowerment and advocacy
- Conduct outreach and engagement efforts
- Coach and encourage consumers in self-advocacy and self-management
- Provide frequent contact and monitoring of progress
- Accompany consumers on clinical and supportive service appointments
- Assist other program staff during some client interventions
- Participate in case conferences
- Assist with maintenance of binder/client files
- Report participant progress to Program Supervisor and others as identified by the participant
