Developmental Pathways (DP) is seeking Case Management Specialists (CMS) in our Systems Navigation Department. Our Case Management Specialists will provide professional-level administrative support to a variety of roles and work processes. These positions will develop and maintain systems of organization, distribution, and communication. Our Case Management Specialists will provide additional case management support to our Case Managers including implementing, promoting, and maintaining effective and efficient processes and operations across departments. These positions will also assist and support staff with the administrative executive of their job duties.

This a 40-hour week, hybrid position, working primarily remotely but coming into our headquarters office in Aurora, CO, and/or other offsite locations as needed. Our ideal CMS will have strong organization, collaboration, critical thinking, and communication skills.

Requirements

  • High School Diploma or GED
  • Personal vehicle in good operating condition for use during work as appropriate
    Valid Colorado motor vehicle license and proof of motor vehicle insurance
    Ability to meet our driving requirements - no suspensions/ no more than two tickets in the past three years

Preferred

  • Associates Degree in business equivalent education and experience
  • Three (3) years of administrative support or customer service experience
  • Experience in human services or nonprofit fields
  • Knowledge of Colorado Department of Health Care Policy and Financing and Colorado Department of Public Health and Environment
  • Bilingual – Spanish/English

Additional Responsibilities

  • Review and oversee case documentation of all annual Service Plans (SPs) and SP revisions to ensure all state and federal requirements are met and all data is accurate
  • Enter accurate documentation into the state-held billing system so that providers can appropriately bill and receive payment for services rendered
  • Ensure the accuracy and timely entry and distribution of written SPs and addendums to appropriate parties for billing and service provision purposes
  • Develop, distribute, and manage SP, and billing data for service providers; enter SP data into state databases; support and resolve billing issues
  • Research participant service utilization and previously provided services and supports and follow up as appropriate
  • Research participant Medicaid coverage and provides administrative support with waiver enrollments; submit the appropriate paperwork to Medicaid for waiver enrollments and ongoing Medicaid coverage
  • Analyze work completed to identify and report on trends
  • Provide professional-level feedback and follow up with other professional-level staff regarding mistakes or missed steps in Department processes and procedures
  • Calculate all support levels and communicate changes to providers, case managers, and other pertinent staff
