An industry pioneer who has been providing community-based services to vulnerable populations for more than 120 years is looking for case managers to support clients with severe and persistent mental health challenges. Rather than working with clients in an office-centric setting, you would be meeting them primarily in community-based settings, providing the most direct access to services possible.

Community Support Case Manager Responsibilities
  • Provide case management services to adults with psychiatric disabilities that encompass all aspects of their wellness and care
  • Provide a combination of face-to-face and phone outreach based on clients’ needs
  • Meet with clients at home and in the community on a regular basis
  • Support clients in accessing services such as therapy, medication management, and primary care, coordinating with psychiatric and medical providers as needed
  • Assist clients with scheduling medical appointments and provide transportation as needed
  • Respond to emergency situations and provide crisis intervention as needed
  • Attend daily team meetings focused on how to best support clients

Community Support Case Manager Qualifications
  • A minimum of an Associates in a human services-related field (Bachelors preferred)
  • A valid driver’s license, reliable vehicle, and ability to pass a driving record check
  • Professional experience is a big plus, but willing to train the right candidate
