Education and Experience Requirements

  • Bachelor’s degree in a human services or related field is required, and a master’s degree or degree candidate from an accredited program is preferred (the degree to be completed within a reasonable amount of time).
  • Must have a valid driver's license, an excellent driving record, and access to a safe, reliable, insured vehicle.
  • The ideal candidate will be licensed or working towards their licensure, in social work, psychology, counseling, or a related field.
  • Minimum of 2 years of experience working with individuals with serious and persistent mental illness.

We are seeking a passionate mental health professional to fulfill the role of Case Manager, who will provide clinical and human services support at our Second Spring South location in Williamstown. The ideal candidate will have exceptional interpersonal and communication skills; and a commitment to promoting a resident-centered, collaborative approach to care operating within a person-first philosophy.

This position is full-time, exempt with a schedule of Monday-Friday 9 am-5 pm.

This position is responsible for providing rehabilitative and social work services to persons with serious and persistent mental illness in a recovery-based and trauma-sensitive environment. Also responsible for coordinating services with auxiliary programs, psychiatric providers, agencies, and the community. Services would be for consumers in a community recovery setting who might generally receive services in a hospital environment. This position is responsible for proactively and positively supporting the forward movement in the organization’s commitment to our mission, recovery principles, provision of trauma-informed care, cultural competence, and building a learning culture

Essential Job Functions: (Include but are not limited to)

  • Assist residents with daily needs including medication, transportation to appointments and events, nutrition, housekeeping, and personal care.
  • Supporting residents following CSC rules and expectations.
  • Participate in continuous learning opportunities.
  • Complete administrative tasks including maintaining timecards, submitting EHR notes and incidents, required paperwork, etc.
  • Provide emotional support individually and in group and family settings.
  • Facilitate repair work with residents as needed.
  • Advocate for clients.
  • Have awareness of clients’ progress and response to medications to provide the program psychiatric staff with information useful in administering effective chemotherapy.
  • Conduct assessments for residents as needed.
  • Provide support and psychoeducation to clients including but not limited to: WRAP, outreach visits, supportive counseling, crisis interventions, assistance with independent living skills, and transportation.
  • Attend group meetings to support Recovery Support Specialists (RSS) and Clinical Paraprofessionals (CPP) in understanding and implementing treatment plans.
  • Empower clients by collaborating with them to develop treatment plans, learn the skills they need to live in the community, and progress toward discharge.
  • Implement a recovery-centered modality by ensuring consistency in provider/resident collaboration.
  • Collaborate with a multidisciplinary team of nurses, direct care staff, therapists, and other professionals in the interest of client advocacy, recovery, continuity of care, consistency, and empowerment.
  • Accompany residents to appointments in the community (court hearings, medical appointments, etc.) and visit residents in the hospital as needed.

Environmental Factors

Duties are performed in a variety of settings (i.e. community, residence, CSC). There is direct interaction with mentally ill adults and/or individuals with substance abuse issues who are experiencing a significant mental health crisis. Private means of transportation, a valid driver’s license, and automobile insurance are required.
Interaction with clients poses some risk of physical injury or property damage. An incumbent may encounter situations that may elicit high levels of stress. Crisis situations can occur at any time. Evening, overnight, or weekend work may be required. Must be able to work independently, and use sound judgment and critical thinking skills.

Confidentiality

Must adhere to and respect all policies regarding resident rights, anonymity, and confidentiality of all residents past and present and other proprietary information regarding CSC. This covers any written or verbal communications regarding a client’s identity, address, nature of their problems, and proprietary information of CSC. Confidentiality regarding substance abuse is mandated by federal law.
Confidentiality policies and procedures remain in effect while employed by Collaborative Solutions Corporation and once employment is terminated.
