Case Manager Job at Crawford County Mental Health Center
Crawford County Mental Health Center is taking applications for a Full-Time Community Based Specialist position. This position provides support services to severe and persistent mentally ill (SPMI) adults in a one to one setting.
Position requires Bachelors degree in Human Services Field and/or relevant experience may be substituted for degree requirement. Must have valid drivers license and ability to move freely around the community. Must be able to meet productivity standards and documentation requirements. This position involves assisting with reintegrating adults into the community, support, role modeling, advocacy, and facilitating independent living. This position may involve non-traditional hours such as nights and weekends.
