Healing Hands & Hearts Inc. (3H) has full and part-time positions for Case Managers (MHS/MHP). This person will provide Mental Health Services to adults and children within their home environment and a community setting, in and around the Alexandria.Also, looking to add qualified team members in and around the Leesville and Marksville areas. This position utilizes a variety of direct care skills in the assessment and treatment intervention services for children, adolescents, adults, and/or families in behavioral health care. This role will manage and execute strategies to improve client well-being.

QUALIFICATIONS Required:

  • Master/Bachelors of Science degree from an accredited college or university with a MAJOR in the field of: Social Work, Counseling, Psychology, or Sociology.**
  • Master/Bachelors of Science degree from an accredited college or university with a MAJOR or MINOR in the field of: Rehabilitation Services, Special Education, Early Childhood Education, Secondary Education, Family and Consumer Science, or Human Growth and Development.
  • Provide direct services including Community Psychiatric Support Treatment (CPST), Psychosocial Rehabilitation (PSR), and Crisis Intervention (CI) to recipients and their families in the MHR program.
  • Ability to set your own schedule
  • Valid driver’s license in good standing
  • Car registration and vehicle insurance
  • Successful clearance of local and/or state background checks
  • Successful clearance of drug and TB test

DUTIES AND RESPONSIBILITIES:

  • Assists in maintaining all new and concurrent authorizations on each client in their caseload while ensuring timely submission of re-authorization requests
  • Provides face-to-face contact with client caseload on a regular weekly basis
  • Makes visits to schools, homes, and community to assist the client with ongoing treatment goals. Provides needed transportation to and from the office for appointments
  • Attends all mandatory training and meeting
  • Coordinates discharge planning and community re-entry following hospitalization, residential services, and other levels of care
  • Provides Psycho-education and training to family, unpaid caregivers, and others who have a legitimate role in addressing the needs identified in the ISRP or treatment plan
  • Provides direct preventive and therapeutic interventions that will assist with skill building
  • Educates clients and collateral on relapse prevention and disease management strategies
  • Provides career guidance, as needed
  • Provides ongoing symptom monitoring and management
  • Provides ongoing medication monitoring with a report to medical providers
  • Assists with assuring compliance to all applicable rules, regulations, policies, and procedures of the agency and governing standards
  • Assists with working with the mental health agency to solve day-to-day problems regarding clients and/or their families
  • Ensures all paperwork is submitted promptly to LPC and/or upper management
  • Assists with implementing, monitoring, and participating in quality assurance and improvement program
  • Participates in treatment team meetings along with other team members
