ESSENTIAL FUNCTIONS:
  • Participate in the initial evaluation process, discharge planning, family interventions and disquisition of treatment plans as delegated.
  • Ensure the interdisciplinary treatment plan reviews reflect current patient management, treatment changes and patient behavior.
  • Maintain close communication with clinical staff, case manager, GPS, etc.
  • Maintain a healthy therapeutic environment for patients.
  • Complete patient evaluation no later than24-48 hours after admission.
  • Ensure all documentation is complete and accurate.
  • Recognize situations that require specific intervention from other disciplines such as nursing, nutrition, occupational and recreational therapy, etc., and channels these with the indicated staff.
  • Document initial note of assigned patients no longer than 24-48 hours after admission.
  • Review and analyze data obtained in comprehensive evaluation of patients in your care, interpret these for the purpose of implementing a treatment plan to follow.
  • Identify strengths, needs, and problems to be included in PTM and work during hospitalization.
  • Identify specific patient needs that impact patient's discharge and location planning, using findings identified in comprehensive evaluation and initial interview.
  • Identify needs related to patient/family education relevant to the discharge process, home patient management, patient location in community, medication, etc. Document these in PTM and in a discharge planning note.
  • Ensure the master plan justifies and fully demonstrates the patient's stay at this level of care and includes specific recommendations for their next level of care.
  • Discuss treatment plan with patient and/or family according to the stipulated time.
  • Modify treatment plan as needed with events and changes that may occur without waiting for the stipulated date for review.
  • Learn relevant aspects of the patient's medical coverage, identifying service levels that your plan covers days of hospitalization, hosp. partial, tx. outpatient, among others.
Document all activities related to discharge planning, family interventions, etc., in the patient's clinical record.
  • Maintain effective communication with referral sources, external auditors, offering information relevant to the treatment and planning of patient discharge.

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
  • Master's Degree in Social Work with at least 1 year of experience in direct treatment with psychiatric patients required or;
  • Master's Degree in Psychology, preferably clinical or at least two (2) years of graduate studies towards master's degree in Clinical Psychology required.
  • Two years of experience in mental health facilities working directly with psychiatric patients preferred.
  • Experience in group therapies, crisis intervention, discharge planning and family management, specific knowledge about the Mental Health Act and procedures for involuntary admission.
  • Knowledge of law related to Law 54, etc., and its applicability in this environment.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
  • Licensure or certification for counseling, as may be required by the territory in which the facility operates.


Experience

Required
    • Master's Degree in Social Work with at least 1 year of experience in direct treatment with psychiatric patients required or; Master's Degree in Psychology, preferably clinical or at least two (2) years of graduate studies towards master's degree in Clinical Psychology required.

Education

Required
  • Masters or better in Human Services or related field
  • Masters or better in Psychology or related field
