Position Title: Case Manager I

Department: Community Support –Housing Navigation Services

Reports To: Program Operations Supervisor: Community Supports-Housing

SALARY RANGE:

$24.75 - $28.50 per hour DOE

POSITION SUMMARY: LANCASTER, CA. SITE

Provides housing case management services to patients assigned to individual caseload. Position will conduct onsite and /or in field-based advocacy, outreach, engagement, re-engagement, and housing navigation services including outside linkage to resources.

Housing Transition Navigation Services include:

  • Conducting a tenant screening and housing assessment that identifies the Member’s needs, preferences and barriers related to successful tenancy. The assessment may include collecting information on the Member’s housing needs, potential housing transition barriers, and identification of housing retention barriers.
  • Developing an individualized housing support plan based upon the housing assessment that addresses identified barriers, includes short- and long-term measurable goals for each issue, establishes the Member’s approach to meeting the goal, and identifies when other Providers or services, both reimbursed and not reimbursed by Medi-Cal, may be required to meet the goal.
  • Searching for housing and presenting options.
  • Assisting in securing housing, including the completion of housing applications and securing required documentation (in other words., Social Security card, birth certificate, prior rental history).
  • Assisting with benefits advocacy, including assistance with obtaining identification and documentation for SSI eligibility and supporting the SSI application process.
  • Identifying and securing available resources to assist with subsidizing rent (such as Section 8 state and local assistance programs, etc.) and matching available rental subsidy resources to Members.
  • If included in the housing support plan, identifying, and securing resources to cover expenses, such as security deposit, moving costs, adaptive aids, environmental modifications, moving costs, and other one-time expenses.
  • Assisting with requests for reasonable accommodation, if necessary.
  • Landlord education and engagement.
  • Ensuring that the living environment is safe and ready for move-in.
  • Communicating and advocating on behalf of the Member with landlords.
  • Establishing procedures and contacts to retain housing, including developing a housing support crisis plan that includes prevention and early intervention services when housing is jeopardized.
  • Identifying, coordinating, securing, or funding non-emergency, non-medical transportation to assist Members’ mobility to ensure reasonable accommodations and access to housing options prior to transition and on move in day.
  • Identifying, coordinating, environmental modifications to install necessary accommodations for accessibility.

CATEGORIES OF DUTIES

General Duties:

  • Conducts self in a professional manner in areas of dress, language, attitude and behavior.
  • Exhibits full knowledge and support of the philosophy and objectives of the Agency.
  • Fully understands and maintains policies regarding professional ethics, including
  • Inform patients about other services offered by Tarzana Treatment Centers, Inc. (TTC) that may be appropriate for the patient’s family members.
  • Provides regular, comprehensive progress note charting in patient’s Medical Record which document contacts, progress towards identified S.M.A.R.T goals, program participation, referrals, discharge planning and other case management activities.
  • Critical thinking using a client center approach, motivational interviewing, and trauma informed communication
  • Attends in-services trainings and educational workshops.
  • Attendance required for all staff meetings.
  • Cooperates and communicates fully with other staff members regarding patient behavior, progress activities, etc.
  • Works closely with other multi-disciplinary team members to develop a comprehensive Individualized Treatment Plan on assigned patients and to ensure that all identified problems are addressed.
  • Assist with employment/vocational referrals, provide vocational counseling re resume preparation, job interviewing etc.
  • Provides outreach and follow up services to patients in and out of treatment, to check progress, offer additional services, help reengage in treatment, make needed referrals.
  • Facilitates discharge-planning efforts, commencing at time of admission on all assigned patients, including areas of housing, employment, medical psychiatric, financial, aftercare, etc.
  • Makes direct contact within agency and with outside entities as appropriate, in order to assist patients in resolution of legal, financial, housing, medical, psychiatric and discharge planning issues
  • Performs other duties as assigned by Operational Supervisor

EXPERIENCE

Case Manager I - Completion of an academic Addiction Studies program and certification with ACADC, CCBCDC, BI, FACT, IACC or CAARR

  • Or Bachelor’s Degree and registration with one of the 10 DADP approved state level certification programs. Must obtain certification within 5 years of employment.
  • 3 years Case Management experience

Case Manager II - Completion of an academic Addiction Studies program and certification with CAADAC, CAADE or AAHCP

  • or Masters Degree in Behavioral Sciences or in at least 3rd year of a doctoral program in Psychology; or Internship registration with the Board of Psychology as a Psychological Assistant or with the Board of Behavioral Sciences.
    • If not licensed after three (3) years of employment must show evidence of enrollment in Addiction Studies Program and register with one of the DADP-approved state level counselor certification agencies
  • Must be registered or certified upon hire and no later than six (6) months from date of employment.
  • 5 years Case Management experience

2. For all other Programs:

  • Case Manager I - BA/BS/BSW degree in social work or related fields and 3 years prior case management experience
  • AFSD
  • Case Manager II - Masters degree in Behavioral Sciences or in at least 3rd years of a doctoral program in Psychology and 5 years Case Management experience

3. Bilingual English/Spanish preferred

4. Valid California driver’s license and insurance

LICENSES/CERTIFICATES

  • For AOD programs, registration or certification with a DAPP-approved state level counselor certification agency
