The Case Manager/Clinician is a key associate at Hope Network whose primary responsibilities include: Coordinate all services necessary to maximize child’s success in treatment and growth in program. In residential settings, this position provides clinical and program consultation to direct care staff. Trauma informed approach is necessary. Ensure case management compliance with agency standards and the practice model.

Summary:

1. Regular and predictable attendance is an essential requirement of this position.

Clinical Responsibilities:
1. Provide comprehensive psychosocial assessments.
2. Perform individual and group therapy that is suited to the needs of each child. A minimum of 0.5hrs of individual therapy and 2hrs of group therapy are required.
3. Meet with all kids on case management case load at least weekly. A minimum of 0.5hrs of case management contact is required.
4. Coordinate and ensure child involvement in the Person Centered Planning Process (PCP) for assigned consumers. Assure responsibility for the development & implementation of each assigned child’s Individual Plan of Service.
5. Link children to necessary ancillary services.
6. Act as an advocate and/or a liaison coordinating services among providers of service and referral source.
7. Maintain children’s records – meeting all appropriate standards according to HNBHS and all regulatory bodies.
8. Participate in legal proceedings including petitioning and testifying in clinical emergencies.
9. Facilitate the coordination of children’s intakes, discharges, and transfers to and from other facilities programs, agencies.
10. Ensure that children are transported to appointments, accompanying them when medically necessary. Obtain relevant medical/treatment information from appointments.
11. Link and coordinate follow-up of professional recommendations.
12. Facilitate communication with guardians, payees, and family members with regard to children’s care, financial and treatment issues.
13. Meet with assigned children face-to-face as medically necessary in their Individual Plan of Service.
14. Provide after hours on-call services as designated by HNBHS.
15. Facilitate hospitalizations as needed and monitor progress by attending hospital medical/team meetings and coordinating with hospital social worker.
16. Provides consultation to direct care staff regarding clinical and program issue, where applicable.
17. Participate in children’s program reviews and team meetings, where applicable.
18. Provide coverage for case-managers on PTO as needed.

Administrative Responsibilities:
1. Participate in committees, task forces, and other related meetings and activities as assigned by supervisor.
2. Practice according to licensing and accreditation for assigned program(s).
3. Attend agency meetings: clinical, administration and management.
4. Network with relevant agencies and providers as needed.
5. Follow general safety practices. Report any unsafe conditions to the manager of that location.
6. Complete and submit Service Activity Logs (SAL’s) in accordance with agency policy/protocols.
7. Coordinate benefits and verify benefit status monthly.
8. Other duties as assigned by supervisor.

Requirements: Educational / Talent Requirements:
1. Minimum Education required: Master Social Work Degree, Master Counseling Degree, or Master Psychology degree and eligible for licensure.
2. Valid driver's license with acceptable driving record according to Hope Network insurance carrier definitions.
3. Must complete a Central Registry Clearance form for the State of Michigan and any state in which residence and/or employment was previously established. Michigan Central Registry Clearance must be completed on an annual basis.
4. History of working with children with intensive therapeutic and behavioral needs is highly preferred.
5. A comprehensive knowledge of Trauma-Informed Care and Gentle Teaching foundational principles and experience implementing and working within these frameworks is preferred.
6. Experience, comfort, and proven effectiveness leading trainings and treatment-based discussions amongst a variety of staff, including clinical, administrative, and direct care, is preferred.
7. Experience and knowledge working within the Michigan Children’s Foster Care system as well as the Michigan Department of Health and Human Services is preferred.

Work Experience Requirements:
1. Prior case management experience in a mental health or rehabilitation setting preferred.
2. Two years working with children in the foster care, mental health, or residential care, preferred.
3. Ability to communicate effectively in both verbal and written formats, to meet position requirements.
4. Ability to articulate and actively support the Hope Network mission to various audiences.
5. Ability to work autonomously and as a team.
