Overview:
Serves as liaison and coordinates care for patients with biopsychosocial needs in the context of medical, emotional and/or behavioral problems. Assesses for such needs at the individual level, as well as facilitating and tracking successful referral to resources. Participates in population-based Care Management leading to the integration of primary and mental health care to treat the whole patient.
Responsibilities:
Daily Job Duties:

  • Assist clients and/or families, both in person and virtually, in identifying and accessing community resources to alleviate social, environmental, and/or economic problems impacting health care needs.
  • Coordinates, manages, and tracks all aspects of patient referral process and adheres to appropriate timelines and organization policies.
  • Assists families in understanding the implications and complexities of their medical condition and its impact on lifestyle.
  • Serves as a liaison and/or coordinates care between the client’s providers, other treatment providers, community groups, and social service agencies
  • Maintain detailed current referral information and make referrals
  • Refers clients and/or their families to community resources (programs, agencies, other providers, etc.) to assist in alleviating social, environmental, and economic problems affecting health care needs
  • Update and maintain resources and contact points for providers as needed
  • Other duties as assigned

Compliance/Training:

  • Maintain patient care standards following established LSCC health education and information guidelines
  • Participate in data collection, focus groups, TJC, PCMH, and other quality improvement initiatives
  • Assume responsibility for his/her own professional development and for sharing knowledge with others
  • Invest own time and effort to improve knowledge, skills and judgment
  • Responsible for knowledge of and compliance with all LSCC policies and procedures
  • Maintain professional manner and appearance


Qualifications:
REQUIRED EXPERIENCE, EDUCATION, AND LICENSE:


  • Graduate of a bachelor’s level social work program from accredited college or university with a CCM (Certification in Case Management) OR
  • Minimum one (1) year experience as social worker or therapist, or case manager preferably in a community-based and/or medical setting
  • Basic Life Support (BLS) certification issued by the American Heart Association (AHA)


PREFERRED EXPERIENCE, EDUCATION, AND LICENSE:

  • Master’s degree in Social Work with CCM (Certification in Case Management)
  • Experience working in behavioral health and/or human services
  • Proficiency with MS Office Suite (Word, Excel, and Outlook)
  • Proficiency with Electronic Medical Record (EMR) system, as well as computer and web-based interfaces
  • Bilingual English/Spanish language skills
