YOUNG MENS CHRISTIAN ASSOCIATION RARITAN BAY AREA Perth Amboy, NJ
Under the guidance of the Clinical Case Manager and the Program Director, the Case Manager/Outreach Worker is responsible to develop plans and identify family and individual strengths and needs; provide information on community resources and facilitate access to resources; follow-up on referrals to ensure that linkages are successful; support therapeutic interventions in everyday practical experiences of individuals and families living at the YMCA’s residences.
Requirements:QUALIFICATIONS
A. Experience:
3-5 years related experience.
B. Education/Certification
Associates degree (or combination of lived and professional experience) from an accredited university or college in counseling or related field.
C. Skills:
- Bilingual- English/Spanish both oral and written.
- Ability to carry out instructions independently.
- Ability to get along with people of diverse backgrounds.
- Ability to diffuse conflict effectively when necessary.
- Skills in planning, organizing, systematizing, implementing are essential.
- Must have proven skills in problem solving and decision-making.
Interpersonal Skills:
- Familiar with the culture and mission of the organization.
- Excellent communication and organizational skills required.
- Friendly and polite. Ability to negotiate and manage interactions with employees, coworkers, department heads, and vendors as needed.
- Ability to be gracious with all persons of various ages, cultures, and creeds.
