Introduction
The incumbent shall be sensitive to the diversity of Native cultures as well as have knowledge of the disease of alcoholism/substance abuse issues and the ability to provide and coordinate direct aftercare activities is critical. Must be sensitive to gender specific, gender diversity and sexual orientation needs and issues.
Requirements
Case Manager I – Certified Alcohol and Drug Counselor Registrant (CADC-R)
TL 115: High School diploma or GED and one year of verifiable experience in a human services field. Must be able and willing to become certified, within 18-months, as a Certified Alcohol and Drug Counselor through the Mental Health and Addiction Certification Board of Oregon (MHACBO) that includes the completion of 1000 hours of supervised experience by an Outpatient Counselor III and 150 hours of Alcohol and Drug Education.
Case Manager II – Certified Alcohol and Drug Counselor 1 (CADCI)
TL 117: The Case Manager I transitions to a Case Manager II after obtaining CADCI certification through MHACBO with 2 years of experience in the Behavioral Health (BH) field, satisfactory evaluations, and approval from the BH department Administrator.
Knowledge of: Tribal and other services available to assist the person in recovery. Substance use disorders, mental illness, socioeconomic and cultural factors. ASAM criteria. Ability to: Work with a wide variety of people, and communicate effectively with program staff, clients, and the community. Demonstrate cultural competency in direct service, within the organization, and in program planning and development. Network effectively with Tribal and community agencies and the multidisciplinary team. Ability to prioritize both independently and as a member of a multidisciplinary team. Other: Abstinence during employment in this capacity is essential to the overall effectiveness of the program. Must possess First Aid and CPR certification, or must acquire within 30-days of hire. Must possess a valid Oregon State driver’s license, insurance and be eligible to drive a GSA vehicle.
Duties:
Provide skill building for independent living, physical, and recreational activities. Participate with clinical staff in planning and providing aftercare services. Participate with clinical staff in making referrals to tribal and other community resources and twelve step groups leading to self-sufficiency. Prepare and complete program reports. Update records to document client services delivered. Maintain up-to-date client records according to Indian Health Service, state, and program standards. Maintain intake packets, including updating forms and releases within ReliaTrax software program. Gather documents and complete intakes for A&D counselors in Behavioral Health and the Medically Assisted Program (MAT), as appropriate. Ensuring documentation in file/computer is up to date. Collaborate with Peer Recovery Mentors/A&D Counselors to track needs of potential individuals coming out of incarceration. Contact residential treatment facilities to determine bed availability. Track completion dates and assist the team with managing aftercare plans for clients who are at detox and residential facilities. Assist clients with over- the- phone screening process for detox. Arrange and assist with transportation as needed. Help facilitate logistics of outdoor adventure based activities. Complete reports for Hope and Drug Court on a weekly basis, entering data within the specific County data system. Observe, collect and track UA’s for clients, and specifically tracking UA’s for drug and hope court participants, and ensures information gets to drug court on a timely basis weekly. Gathering reports from A&D Counselors and Mental Health Therapist to forward to other partners (e.g. 477 Self Sufficiency, Vocational Rehab, courts, probation, ICW, DHS….). Attend monthly meetings with other partners (e.g. 477 Self Sufficiency, Vocational Rehab, courts, probation, ICW, DHS…..) as requested. Attend weekly Medication Assisted Treatment (MAT) staff meetings to discuss mutual clients, assisting with coordinated care for dual Behavioral Health/MAT clients. Assess perspective patients for treatment, identifying psychosocial, mental health and medical issues through a face-to-face screening upon admission. After receiving credentialing as a CADC-I, complete assessments of Behavioral Health and MAT clients to determine an initial level of care (LOC) the client meets using American Society of Addiction Medicine (ASAM) criterial and medical necessity guidelines. Assist in the process of referring out clients that do not meet appropriate criteria for admission. Monitoring of pre—phase requirements for Treatment Court and reporting necessary documentation in adherence to the Lincoln County Treatment Courts SCMS system. Review and understand insurance information provided by the in-patient admissions office, clarifying which benefits are available to the client. Serve as the client advocate to get needs met related to intake and assessment. Work collaboratively with the Behavioral Health, MAT, Medical, Prevention and Mental Health staff to verify the client’s needs are being met. Provide appropriate communications with tribal and community partners, such as Lincoln County Parole & Probation or other referral agencies regarding client compliance. Facilitate groups when counselors are unavailable. Facilitate at least one Positive Indian Parenting 8-week group annually, for Behavioral Health and others involved with tribal programs. Assist with gathering statistics for the Quarterly reports. Other duties as assigned.
After the six-month probation the employee will be evaluated of his/her performance and at that time, it will be, decided if the employee is entitled to a regular employee status.
