Purpose:

*The ACT Program is hiring multiple positions as Case Manager for our Youth ACT team and our Adult ACT team.*

This position is responsible to help individuals who have serious and persistent mental illnesses and/or substance disorders live self-directed and satisfying lives by providing services directly, minimally using outside resources and diverting hospitalizations by finding alternative interventions whenever possible. This position provides psychiatric rehabilitation, service coordination, and support in accessing available benefits and community resources. When there are other providers, there is close coordination and collaboration with these other outside resources to ensure the best possible service.

Key Responsibilities:

  • Assess needs, identify goals, and plan service so that participant's needs are met and goals are enhanced. Provide psychiatric rehabilitation including teaching, modeling, coaching and education for life skills and mental health and symptom management.
  • Complete assessments, individual treatment plans and supporting documentation.
  • Communicate consistent information and offer assistance to the team to ensure continuity of service for the clients served.
  • Ensure services are documented in individual records so information is readily available, results of services can be determined, and record-keeping standards are met.
  • Provide flexible individualized support so clients can lead self-directed lives.
  • Access available community and housing resources so that necessary and relevant resources match client needs and desires.
  • Facilitate communication among others involved, to provide coordination and define service direction to attain clients' goals.
  • Provide crisis assessment and intervention so clients receive the assistance they may require to be safe in their home and in the community.
  • Provide on call crisis support 24/7 in weekly intervals, provide coverage for evenings, weekends as holidays as determined by team needs.

Education and Experiences:

Required:

The Case Manager shall be qualified under Minnesota law as a Mental Health Case Manager as follows:

  • Holds a Bachelor's Degree in one of the behavioral sciences or related fields from an accredited college or university and has 2,000 hours of supervised experience in the delivery of services to persons with mental illness.

Or

  • Is qualified as a Mental Health Practitioner with 4,000 hours of experience providing services to adults with mental illness, substance use disorders or children with emotional disturbances.
  • Have completed 30 credit hours in relevant course work and 2,000 hours of experience providing services to adults with mental illness, substance use disorders or children with emotional disturbances.

Preferred:

  • Master's degree in psychology, social work or related field.
  • Licensed as a Mental Health Professional in the state of MN or Clinical Trainee pursuing License.

Other Required Qualifications:

  • Must be ambulatory and able to negotiate stairways.
  • A valid Class ‘D’ Driver’s License, reliable vehicle, satisfactory driving record and necessary insurance.
  • The ability to lift and carry heavy weights that could be up to or even exceed 40 pounds.
  • Depending on position the ability to provide coverage on evenings and weekends.
  • The ability to carry a cellphone and respond to calls after hours for crisis intervention, collaboration and collateral information 24/7 in weekly intervals in a rotation with other team members.
  • Must not be actively receiving services from Guild and must not have received services within the last two years.
