Job Title: Specialized Family Support Program (SFSP)
Job Description:
Responsible for a case load of clients referred by the Department of Children, Youth and Families. Collaborate with SFSP Clinical Supervisors to assess the clients’ and families’ needs; coordinate the activities that support all aspects of development, implementation, monitoring and modification of treatment plans for SFSP clients. Case Managers provide transportation for children enrolled in SFSP.
Education, Experience and Special Skills:
- Bachelor’s degree in Social Work, Mental Health Counseling, Behavioral Health or in a related field with years of experience.
- One-year experience of working with child welfare preferred.
- Knowledge and skill in developmental disabilities, family assessment, functional behavioral analysis, positive program development, negotiation skills, collaboration, problem solving and decision making.
- Ability of work flexible evening hours.
- Maintain a valid driver’s license, current automobile insurance and a driving record that meets the guidelines and requirements of the organization. Has reliable transportation.
