General Summary:
Provides diabetes education services for primary
care services and education consultation of patients. Serves as a resource to
the clinical team, members of the Department of Medicine, Dentistry,
Obstetrics, Infectious Disease, Mental Health and to nursing staff in this
specialty area.
Education:
Bachelor’s degree in nutrition or nursing
Experience:
Approximately three to four years of
progressively more responsible job related experience in diabetes education in
a formal diabetes program. Knowledge in diabetes theory practice and education
in order to develop educational programs, conduct patient assessments, and
develop complex plans of care
Certifications/Registration/Licensure:
Licensure as a diabetes educator or certification
eligible as a diabetes educator are required. If hired as certification
eligible, employee must obtain certification within twelve months of hire date,
failure to obtain certification within 12 months will be grounds for
termination.
