Sangre de Cristo Community Care is looking for an experienced Certified Nursing Assistant to join our team in La Junta, Colorado. Responsibilities
- Provides patient care as directed by the RN or Case Manager.
- Participates actively as a member of the Interdisciplinary Team to assist in the development and implementation of the patient’s plan of care.
- Provides a full range of personal care through regular visits in patient’s home and in facilities in accordance with hospice standards and the CNA scope of practice.
- Aid with personal hygiene, ambulation and exercise, and nutritional support for patient respecting personal and professional boundaries

Requirements:Must be at least 21 years of age.
A valid non-restricted CNA license in the state of Colorado is required.
Must have a valid non-restricted driver’s license.
Flexibility and availability to work a weekend schedule if needed.
At least 1 year of hospice or home health experience preferred.
Must comply with all mandates and organizational policies regarding vaccinations to include annual influenza and Covid19
