Valley Community Services Board is looking for a Child Mental Health Case Manager to join our team! As a member of the highly collaborative Community-Based Services team, the Child Mental Health Case Manager will play a vital role in providing and connecting children and their families to support services.

Essential functions of the position include:
- Provide intensive case management services to SED & at-risk children, and adolescents.
- Collaborate with the children, adolescents, and their families, to facilitate adjustment at home, in school, and in the community.
- Help children, adolescents and their families navigate crisis stabilization and emergency intervention services as needed.
- Develop and complete Individual Service Plan (ISP) reviews on caseload in accordance with agency standards and Medicaid criteria.
- Coordinate care and discharge planning with the Commonwealth Center for Children and Adolescents (CCCA).
