The Canyon at Santa Monica
is located three and a half miles from the beach in the heart of West Los Angeles; we provide an exquisite healing oasis for individuals seeking recovery in an outpatient setting.


The Client Care Coordinator II assists clinical team with all aspects of client care coordination within the program with a focus on discharge. Connects patients to community resources (employment, volunteer opportunities, school). Ensures regular follow-up with client’s outside collateral (sober livings, courts, employers etc.).


Key Responsibilities include:
  • Integrates Patient Centered Care into all phases of treatment and in interactions with patients and their families.
  • Utilizes FRN endorsed client-centered therapeutic interventions and rapport-building practices, including motivational interviewing techniques, to non-confrontationally enhance consumer insight about their participation in the program and their plans for discharge.
  • Provides evidence based clinical practice techniques and 12 Step Orientation.
  • Assists clinical staff with the intake process.
  • Verification of insurance benefits
  • Assist Primary Therapists with scheduling patient appointments (medical, holistic)

Requirements:

Education: Bachelor’s degree in social work, counseling, psychology or related field required or commensurate experience.


Experience:
Minimum of 3 years’ experience in social service setting or treatment facility strongly preferred.
