Client Experience Navigator (Contractor Position) Job at Haven For Hope Of Bexar County
The Client Experience Navigator will provide care coordination, outreach, and engagement services to address the physical and behavioral health care of Haven for Hope clients. The Client Experience Navigator will focus on responding to the needs of clients from a recovery model, understanding trauma centered services, Person Centered Planning, and motivational interviewing techniques and adherence to Haven’s core values. The Navigator will communicate, collaborate, and assist clients with accessing Transformational services such as housing, employment, and clinical services.
ESSENTIAL DUTIES AND RESPONSIBILITIES.
- Effectively works with Clients, staff, and partner agencies to coordinate care services.
- Monitor areas of the facility to ensure compliance with facility rules, policies, procedures, and protocols.
- Assists Clients with understanding and navigating to available Transformational services offered by Haven for Hope and its partner agencies.
- Identifies problems with service access and utilization providing alternatives to overcome these difficulties.
- Identify and assist Clients with obtaining community resources and services to address the established goals or desired outcomes of Client.
- Ensure all data entry and Client documentation is submitted in accordance with Haven for Hope standards and meets all designated requirements.
- Identify and respond to any Client who is experiencing a problem or crisis to ensure that they are safe and that other service providers are aware of the situation and able to respond with appropriate services.
- Attend regular staff and administrative meetings.
- Other duties as assigned.
EXPERIENCE/EDUCATION.
- High School Diploma or GED
- One-year experience in providing similar services is preferred.
- Preferred experience working with the homeless population is preferred.
KNOWLEDGE/SKILLS/ABILITIES.
Computer Skills
Ability to operate and work effectively in Microsoft Office programs and to complete required data entry in electronic database systems daily
Language Skills
Individual must be able to communicate effectively both orally and in writing. Bilingual (English/Spanish) is preferred.
Reasoning Ability
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists; ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form; ability to handle crisis situations at a mature level.

This position requires that the majority of the work hours be spent walking around and monitoring of three separate facilities, all with multiple floors.
The employee must occasionally lift and/or move up to 25 pounds, e.g. office equipment, file boxes, donations, etc.
Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
