Overview:
This position is for assisting in the coordination of Lakeside Medical Center and Lakeside Provider Practice Quality Performance Improvement activities including, but not limited to, data abstraction for the Joint Commission (TJC), Centers for Medicare and Medicaid Services (CMS) Core Measures, National Healthcare Safety Network (NHSN) measures. Assist with providing data for Leapfrog and other regulatory quality outcomes databases and/or registries. This position assures appropriate data collection, analysis and communication related to changes in definitions, methodology, and mandatory requirements. Primarily responsible for coordination of data with variances and data reporting to the Director of Quality and Patient Safety.
Responsibilities:
Essential Functions:

  • Plans and coordinates special events and meetings as needed (i.e. Professional Practice Evaluation Committee meetings and other Quality related meetings).
  • Assists and participates in activities related to the inspection/investigation of the hospital by regulatory or accrediting agencies.
  • Identifies opportunities for performance improvement based on data trending; recognizing and analyzing undesirable patterns or trends in performance.
  • Assists with providing data for submission to external regulatory agencies. This includes maintaining and providing Medical staff data when required.
  • Maintains and prepares the Ongoing and Focus Professional Practice Evaluations (OPPE and FPPE) for the hospital and Medical Staff Peer Review documents for PPEC meetings.
  • Engages in activities and identify opportunities to improve the quality of care by working with physicians, nurses and other clinicians to develop strategies to improved performance.
  • Works in conjunction with the hospital’s Quality Improvement, Patient Safety, Infection Control, Employee Health, HCAHPS, Sepsis, MEC, Continuous Quality & Patient Safety Committee, Credentialing Department and other assigned committees.
  • Ability to interact with all members of the healthcare team, administrative staff and others in a professional and courteous manner.
  • Ability to build relationships and gain alignment for clinical support from superiors and peers. Other duties as assigned.
  • Maintains complete knowledge and understanding of the hospital’s policies and procedures for the implementation and process to carry out Code Red, Code Blue, Code Yellow, Code A, Code Green ER, Code Brown, Code Pink, Code Orange and Code Purple, Code M, Code Weather Alert of Watch, Code Dr. Armstrong, Cardiac Alert, Rapid Response Team, Hurricane Code and Security Stat Code.
  • Adheres and follows all hospital and departmental policies and procedures.
  • Applies the hospital’s principles of safety in all work practices. Reports any identified safety risks or incidents per the hospital’s policies and procedures.
  • Seeks opportunities to participate in work teams and departmental special projects. Attends and participates in department staff meetings. Attends mandatory department and hospital in-service training sessions.
  • Follows all infection control and employee health policies and procedures at all times.
  • Maintains patient confidentiality at all times in accordance with the hospital’s established policies and procedures.
  • Emergency duty may be required of the incumbent that includes working in special needs or Red Cross shelters or to perform other emergency duties including, but not limited to, responses to threats or disasters, man-made or natural.
Qualifications:
Education:
  • Associate’s Degree preferred.

Experience:
  • Minimum five (5) years of experience utilizing the computer and experience in uploading, analyzing and interpreting process, outcomes, chart auditing and abstraction, performance improvement and data collection preferred.
  • Must be flexible to handle a variable workload, deadlines, and priority changes
  • Must be able to work effectively with management, administration, medical staff, nursing, and ancillary departments
  • Excellent computer skills are required including proficiency in (MS Office (Word, Excel, and Access).
  • Detail oriented organizational skills.
  • Good communication skills both verbal and written.
  • Excellent interpersonal relationship skills with exceptional professional work ethics.
