Clinical Team Leader-Home and Community Engagement Job at Access-Supports for Living

Job Requirements:
This Team Leader will collaborate with Care Management (internal and external), Managed Care Organizations, and Coordinated Behavioral Health Services (CBHS) partners. This position will be responsible for overseeing HCBS services and community engagement of persons entering care in other Access programs who are at high risk, including persons discharged from the hospital.
Primary Functions:
- This Team Leader will carry a small caseload, attend county meetings and market services as well as collaborate across Access programs.
- In this position, the Team Leader will monitor high-risk and high health care utilization registries for Behavioral Health programs.
- This person will be responsible for supervising staff, which may include licensed and unlicensed professionals as well as collaborating with other supervisors to identify staff who can provide services in the community.
- This Team Leader will manage referrals and services in multiple counties and will be responsible for all Home Community services. HCBS services may be provided on and off-site and will include Habilitation, Psychosocial Rehabilitation, Community Psychiatric support, Family Support, and Peer services. These services will be provided in Orange, Sullivan, Ulster, and Dutchess counties.
- This position, as needed, may also provide supervision to small community-based programs that incorporate a clinical component.
- This position will also offer and oversee an array of clinic services in the community.
Qualifications:
- -3+ years working in Behavioral Health Field
- LCSW preferred but will consider LMSW/LMHC
- Strong organizational and time management skills
- Flexibility and ability to motivate others
- Peer experience and/or bi-lingual a plus
