Community Health Licensed Practical Nurse Job at STRIDE Community Health Center
In an aim to help keep our patients, staff, and communities safe, STRIDE requires that all hired employees be fully COVID-19 vaccinated and to obtain an annual flu shot. STRIDE CHC employees are also required to be screened for tuberculosis and to complete a PPD test.
What will you be doing in this role? A successful candidate will:
General Purpose: The Community Health Licensed Practical Nurse (CHLPN) supports the mission of the organization through the provision of high quality, cost-effective nursing care under the supervision of a Registered Nurse or Licensed Medical Provider.
Duties and Responsibilities:
- Participates in the delivery of direct and indirect clinical care within the Licensed Practical Nurse scope and in accordance with organizational policies and procedures under the supervision of a Registered Nurse or Licensed Medical Provider.
- Nursing assignments may include: centralized medication refill processing, telephone advice line, Chronic Care Management, Care Signal remote patient monitoring, Annual Wellness Visits, Care Team integration and support, among others. Assignments may evolve as organizational and patient needs change.
- Collaborates with other departments, external organizations and consultants, including regulatory agencies and coalition partners, to support and coordinate patient care and to maintain STRIDE’s regulatory compliance.
- Maintain patient confidentiality in accordance with Federal and State law and organization policies.
- Document all patient care delivery in an accurate and timely manner.
- Pursue educational opportunities to stay current in the practice of Ambulatory Nursing
- Apply basic time management and patient care coordination skills to effectively provide nursing and health care services.
- Other duties as required.
Job Qualifications
Knowledge, Skills, and Abilities:
- Interact positively and build rapport with patients, coworkers and/or external contacts.
- Work independently and organize work in a manner that ensures accuracy and efficiency.
- Analyze and synthesize data to determine the appropriate solution to the issue at hand.
- Manage multiple projects or competing demands.
- Demonstrate an effective and sensitive response to the needs and concerns of STRIDE’s diverse patient population.
- Handle sensitive information ethically and responsibly.
Education and Licensure:
- Completion of a program of study in Licensed Practical or Licensed Vocational Nursing from an accredited school or training program.
- An active, unrestricted Licensed Practice Nurse license in the State of Colorado or appropriate compact licensing arrangement.
Experience:
- 1 year of full-time experience as a Licensed Practical Nurse in a hospital, nursing home, or community health setting. Previous home health or ambulatory care nursing experience is preferred.
Physical Working Environment:
- Office environment within a clinical setting.
- Enters data into computer programs via computer, mouse and keyboard.
- Moves about the office environment and occasionally to other locations.
- Moves/transports objects up to 25 lbs occasionally.
- Communicates information to others.
- Discerns/analyzes information from others to assist in decision making.
- Works independently.
- Assignments may require working in a community setting such as homeless shelters, outdoors, etc.
- Assignments may also require remote work.
