The Community Health Worker (CHW), under professional supervision, provides mobile connection with physical and behavioral health care. This includes coordinating with the multidisciplinary treatment team as necessary, and facilitating in person visits with individuals. This position is responsible for providing exceptional access to care to communities in need. Due to the changing needs of a community, the day to day activities of this position will vary based on community and agency needs. As a paraprofessional the CHW has an in-depth understanding of the community culture and language in order to provide culturally appropriate health services to the community.

Pay: From $16.00 per hour
