Community Outreach Representative (COR) Job at Health Net of West Michigan
The Community Outreach Representative is responsible for relationship management of community partner and consumer engagement groups. The Community Outreach Representative acts as a liaison between the key stakeholders and Health Net that include: introducing universal screening tools to clinical sites, providing ongoing trainings and orientations for community partner sites, trouble-shooting referral process issues with organizations and medical homes, building strong and meaningful relationships with key stakeholders, attending events and planning outreach activities and interfacing with clients to drive the lived experience consultant initiative. The Community Outreach Representative functions as a member of an interdisciplinary team that provides services to individuals/families by addressing health related social needs to achieve health equity.
The Community Outreach Representative will represent Health Net as a proxy for external agencies and coalitions. They will advocate for Health Net’s interests and advance health equity in local and county-wide collaborative relationships. This position requires the ability to work across the organization in alignment with internal communications, equity, and project coordination efforts. This position supports medical homes, screening and referral processes in collaboration with client services staff, and drives consumer engagement efforts including success stories, focus groups, and client feedback survey data collection and dissemination. Community Outreach Representative position requires the ability to be an “ambassador” for Health Net’s work in the community and build meaningful relationships with persons from diverse cultural, social, economic, and ethnic backgrounds.
The Community Outreach Representative will work as a member of the interdisciplinary Health Net programs team that provides services to individuals/families by addressing Social Determinants of Health to achieve health equity. Those in this position will support the mission, vision, values, and key strategies of Health Net and the organizations that interface with our services.

Pay range: $45,000 - $55,000 per year.
Essential Job Duties and Responsibilities:
- Orient, train and support medical homes and community partners utilizing Health Net services. Monitor satisfaction, troubleshoot issues, and provide solutions.
- Maintain consistent communication and teamwork among main points of contact and other relevant staff; determining issues to be discussed, collaboratively preparing the agenda, and overseeing preparations for the meetings.
- Co-create, implement, and evaluate partner engagement strategies to re-engage CHAP medical practices and maintain relationships with post AHC clinical delivery sites
- Co-develop, implement, and evaluate an ongoing partner communication strategy with CIO and Communications Manager and maintain timely consistent dissemination of all relevant materials and documents
- Ability to report out data results to medical homes and community partners regarding Health Net services and programs.
- Conduct focus groups, client experience surveys, and disseminate findings to organization staff and community partners
- Lead and develop “Lived Experience Consultant” taskforce activities and engagement
- Coordinate and carry-out logistics for meetings, events, and relevant activities for projects/workgroups/coalition activities
- Complete on-going assessment of program operations, and make recommendations for improvement, as needed.
- Build relationships & network with civic organizations, social clubs, businesses, faith groups, and other community organizations that center on the issue of collective community impact. Participate in advocacy for improved access and quality of healthcare for low-income and un- (or under-) insured, by attending coalition meetings and community conversations.
- Represent Health Net at community outreach events and promote the screening, outreach, and navigation services
- Support the mission, vision, values, and key strategies of HNWM and development of collaborative relationships with the organizations that interface with the agency.

Education, Skills and/or Experience:
- High School diploma/GED equivalent required. Bachelor’s degree in health, human services or related field preferred
- Lived experience can be substituted for education requirement
- Relevant work experience in community organizing, relationship building, or fields relevant to assigned responsibilities.
- Time management and ability to multitask and navigate the changing political landscape
- Ability to problem solve and creatively offer best practice solutions to complex issues.
- Proficient computer skills in Microsoft Office computer applications and program software.
- Ability to work with multiple deadlines producing exceptional results.
- Evidence of strong written and verbal communications skills.
- Volunteer management, engagement and retention experience desired
- High-energy person who takes initiative with a positive outlook and can manage ambiguity and change.
- Strong relationship building skills.
- Successful experience working as part of a multidisciplinary team and working effectively with persons from diverse cultural, social and ethnic backgrounds.

Physical Demands/Work Environment:
- Employee is regularly required to sit, stand, walk, reach with hands and arms, talk, and hear.
- Ability to lift and/or move up to 30 pounds.
- Moderate level of stress caused by tight deadlines.
- Moderate level of mental and/or visual fatigue and/or eyestrain may result from looking at a computer screen for extended periods of time.
- Occasional travel is required
- The somewhat open floor plan requires the ability to work in an office environment with moderate noise levels and activity.
