Case Manager - Community Support Specialist

$18.00 hourly rate - 37.50 hours per week - Non Exempt - Monday - Friday

Hours: 8:00 - 4:00 or 9:00 - 5:00 (may require some flexibility with hours)

CHS is currently seeking a full-time Community Support Specialist. The CSS will be responsible for facilitating emergency housing and supportive services to individuals experiencing homelessness residing at Wood Street Commons.

  • Develop a professional and therapeutic relationship with individuals. Maintain and model appropriate personal boundaries.
  • Provide individuals with guidance regarding decision-making. Instruct and assist individuals in learning the skills necessary to complete household duties and teaching them routine healthy daily living self-care skills.
  • Address medical and psychiatric emergency situations, including but not limited to 302 procedures, crisis management, de-escalation strategies, and other non-life-threatening emergencies with clients.
  • Complete required written documentation in a timely manner. Includes incident reports, progress notes, goals of the client, etc. Keep the data base system always updated.
  • Attend necessary trainings and professional development sessions.

Must be Able to Provide:

  • Excellent communication and people skills
  • Exceptional customer service communication.
  • Culturally competent, being able to work effectively with all backgrounds.
  • Certification or training in Crisis Management, mental health, homelessness, and behavioral health education preferred.
  • Ability to work independently.
  • Ability to lift up to 25 lbs.

Qualifications/Required Abilities

  • Bachelor’s in social work, Psychology, or related field, 4 years work experience or combination of work, education and life experiences approved by the agency and funder.
  • Completion of all required training including CPR/First Aid and medication monitoring within first year and maintain afterward.
  • Valid PA driver’s license.
