General Description:
To be responsible for the coordination and implementation of Supported Living Services for people with developmental disabilities. To assist the person and his/her family and Circle of Support to develop and implement personalized support to achieve a better quality of life within the community in which he or she lives. To posses the skills to interact with Regional Center, IHSS and other professional organizations as necessary.


Essential Responsibilities/Duties:

  • To work with people with disabilities, their families and staff within agency resources.
  • To recruit and employ appropriate support staff where and when necessary with the consumers Circle of support involvement.
  • To provide staff training on direct consumer support needs, wants and desires and provide ongoing support and guidance to promote staff autonomy.
  • To assume responsibility for ensuring that staff schedules are maintained at all times to meet the holistic support needs of the consumer.
  • To ensure that direct support staff receive support and regular structured supervision to enhance their professional development.
  • Complete all Initial Training Program courses within 90 days of employment. Ensure that those staff responsible for complete their ITP and volunteer where appropriate to teach or facilitate training opportunities of ITP classes.
  • Be responsible for understanding and communicating JNCSs Mission and all policies and procedures to all staff. Abide by JNCS policies and procedures at all times.
  • Responsible for completing and submitting all required paperwork per policies and procedures and funding source requirements.
  • Attend all mandatory meetings and in-services, including monthly team meetings / quarterly SW meetings and ensure staff attend mandatory training and in-service.
  • Organize and facilitate monthly team meetings.
  • Maintain case management files (Consumer Profile)
  • Maintain individual budgets as per established guidelines and limitations.


Minimal Qualifications
: The following are the minimum qualifications which an individual need in order to successfully perform the duties and responsibilities of this position.

  • A B.A. / B.S. in the related field is preferred, but not essential.
  • A minimum of two years’ experience within a leadership role providing support services to people with developmental disabilities.
  • The post holder should have a sound understanding of the values of inclusion and evidence the ability to support people through a person-centered approach towards meaningful community membership.
  • This position also requires good organizational skills and people skills.
  • The post holder should have good communication skills to develop and maintain appropriate relationships with consumers, their families and JNCS staff, with the ability to work autonomously and be motivated to learn state of the art practices within supported living services.


Knowledge/Skills/Abilities:

  • Ability to communicate clearly, concisely, and persuasively.
  • Ability to use Smart Phone.
  • Ability to interact effectively at all levels and across diverse cultures
  • Ability to be an effective team member.
  • Internalize and implement organization culture and values.
  • Valid Driver’s License, working vehicle, and proof of insurance.
