Summary
To use best practices and technology within a community based residential setting to assist people with developmental disabilities in acquiring the physical, intellectual, emotional and social skills needed to live as independently as possible. Will perform all duties necessary to meet state approved objectives and federal Medicaid guidelines, Department of Human Services and the O.T.C. including the requirement that the employee cooperate with the licensee and Division staff in any inspection or investigation.
Job Description:
- Works a flexible workweek. – Have set schedules except for substitute staff but may require to work in all residential homes. May be asked to work in other homes as needed.
- Assists individuals, as necessary in daily living skills which may require providing feeding, bathing and changing assistance.
- Completes training with individuals towards ISP (Individual Support Plan) outcomes and strategies.
- Successfully documents the progress of all ISP outcomes in Therap each shift for all assigned individuals.
- Successfully completes required residential trainings and DDD and Medicaid trainings, including CPR and First-Aid, HIPPA, Stephen Komninos Law and Danielle’s Law
- Attends conferences, seminars and training relevant to the position.
- Teaching individuals aspects of taking medication, and responsible for administering prescribed medications to individuals served.
- Accompanies individuals to and from appointments and activities.
- Builds a support network between the individuals and community members.
- Accurately completes all in-house documentation in a timely fashion.
- Participates in Circle(s) of Support for personal futures planning.
- Administers and records the administration of medication in accordance with D.D.D. guidelines.
- Work with individuals in connecting with the community:
- Become familiar with and respectful of individuals' interests, goals, dreams, capacities, lifestyle, needs, gifts, and social web (family, friends) by spending time with him/her, consulting with other staff.
- Encourage individuals to explore their interests in ways that build relationships and connect them more deeply to other people in the community.
- Support them in identifying individuals and groups in the community that they could meaningfully participate in.
- Support and coach them in their participation (assist in logistics - transportation, etc.), give counsel on what's considered appropriate behavior within different settings.
- Build bridges - help them to meet others and develop relationships.
- Work with the community in connecting with individuals: Completes all duties as delegated by the Residential Manager and the
- Assist and encourage people from the community to recognize the individual's gifts and to find ways that they can meaningfully participate together; advise on appropriate ways to speak, and respond to the person you're supporting.
- Build bridges - help people in the community to see past the difference and to embrace it; Help dispel ignorance and fear and open doors to friendship.
- Educate the community on the rights and full humanness of people with disabilities; Raise consciousness about the history and current reality of discrimination against people with disabilities, as well as the need and benefit of greater inclusion.
- Facility working and recreational relationships between the individuals and the community.

- Ability to bend, lift and assist in physical transfer of individuals needing assistance.
- Ability to drive large vehicles including non-ambulatory lift vehicles.
Requirements:
- 1 Year of Direct Support Experience
- High school diploma (or GED equivalent)
- Valid driver’s license with a driving record that is acceptable to OTC Motor Vehicle Standards, must be driving for at least 3 years and have a full (not provisional or probationary) license.
- Must be able to effectively communicate both verbally and written
- Must be 21 years of age.
- Valid driver license with acceptable driving record
- minimum of 3 years driving experience
- Acceptable Background Check and Drug Testing
- Must have Covid vaccinations
