Position Summary

Administers and assists with adult day health services (ADHS)/Outpatient Clinic, Medication Services under the licensure of the Arizona Department of Health Services, Offices of Behavioral Health and Assisted Living Licensing. Responsible for coordinating the program including intake and discharge of participants, documentation, monthly reports; clinical supervision of staff, marketing of program, working with the Advisory Council or Board of Directors, working with community agencies and other external stakeholders, coordinating the maintenance of building and equipment. This position is classified as Safety Sensitive.

This position has supervisory direct reports who oversee 10-20 direct reports.

Leadership

Working in partnership with senior leadership, develops strategic alignment plan for one’s assigned program/facility in support of FSL’s strategic plan. With a focus on leadership, mission, culture, strategy, best practices and continuous improvement, performs the following:

  • Leads and directs the program/center’s resource planning and allocation for optimal capacity, maximum utilization, performance, outcomes and overall productivity to include:: ensures staff schedules provide proper coverage, at all timesWorkforce planning and management

: observes and participates in programmatic activities and outdoor education / recreation to ensure consistent application of programmatic treatment program with participantsby direct care workers.Program integrity
ensures supervision of participants in the Center and outside the Center (transit via transportation) to ensure integrity of milieu. Participant supervision:

  • Ensures that programmatic components, quality standards are enforced throughout the program
  • Drives program/center’s financial performance, monitoring and providing monthly analysis on the budget spend
  • Ensures compliance with all federal, state, local, and other applicable regulatory and/or program requirements to include audit readiness.

Talent Management

In collaboration with leadership and Human Resources, develops and executes a plan for retaining and developing talent to meet current and future business objectives to include:

  • Monitoring the engagement levels of direct reports and removing barriers to high engagement and retention
  • Participating in talent review evaluations
  • Collaborating with direct reports on their professional development and growth
  • Assisting with developing leadership succession plans for the department

Supervisory

Carries out supervisory responsibilities in accordance with FSL’s core values, agency policies, procedures, and applicable laws. Responsibilities include:

  • Actively participating in the interviewing and selection processes and ensuring successful functional onboarding and training of new employees
  • Planning, assigning, and directing work for direct reports
  • Setting reasonable stretch performance goals, providing constructive, balanced, regular performance feedback & coaching, and conducting 90-day, quarterly, and annual performance appraisals
  • Recognizing and rewarding performance excellence
  • Communicating and enforcing company policies and programs
  • Applying corrective action, addressing complaints and resolving problems in a timely fashion, involving and collaborating with leadership and Human Resources as appropriate

Functional/Operational

Carries out the following personally or through subordinate staff

Care Delivery

  • Models, works with, and trains staff in the management of challenging behaviors expressed by participants, i.e., wandering, verbal and physical aggression, etc.
  • Monitors and guides team involved with regular care planning conferences to determine participant’s progress toward outcomes
  • Provides oversight of monthly meal planning is ordered efficiently
  • Coordinates recreation, socialization, health, transportation, and social service components of ADHS/Outpatient Clinic Medication Services program to include monthly calendar of events
  • Ensures that weekly menus are posted and have been approved for nutritional value; meals are prepared and served to comply with government regulations
  • Assesses, monitors and directs day to day transportation involving members being transported to and from the Center through FSL’s fleet or through public/private transportation
  • Provides information and referral services to all participants and caregivers Monitors employee hours and signs time sheets

Quality, Compliance & Continuous Improvement

  • Drives Performance and Quality Improvement initiatives at the program and agency level by proving feedback and new ideas for elevating performance improvements
  • Ensures required number of personnel are on duty to ensure participant supervision at all times by managing workforce staffing and schedules
  • Ensures compliance with CACFP programs along with other funding, such as, but not limited to DDD, VA, ALTCS, SAIL, and CDBG
  • Ensures staff attend required training needed for their positions
  • Coordinates team efforts through regular team meetings
  • Responsible for maintaining licensure requirements under DHS
  • Oversees safety committee to ensure that all fire, disaster, and civil disaster drills are completed and documented
  • Complies with government contract regulations and provides monthly, quarterly, statistical and financial reports
  • Engages with community agencies by sitting on committees, researching, and utilizing available services, and assisting in workshops and seminars of other agencies when invited
  • Ensures that all staff are trained in Universal Precautions and that proper infection controls are maintained at all times

Intake & Discharge

  • Reviews and provides decision making on approval of participant intake and discharge including oversight of the assessments of each participant
  • Authorizes mileage reimbursements, maintenance, and petty cash
  • Assists with developing budgets and ensuring forecasting throughout the year aligns with revenue and expenditure trends.

Community Awareness

  • Works with the FSL Marketing and Community Outreach Team to market program(s), according to FSL’s marketing plan
  • Provides Advisory Council or Board of Directors with program information
  • Tracks, monitors, and actively works referral sources with a focus on census and financial stability

Requirements

Education/Experience/Credentials/Knowledge/Understanding

  • Master’s degree (M.A.) or equivalent; or four to ten years related experience and/or training; or equivalent combination of education and experience
  • Minimum of five years of experiencein human services industry, specifically in the administration and operation of a program offering transportation, recreation, social services and health services to high-risk participants and their families
  • Minimum of 3 years’ experience working with older adults, adults with physical disabilities, and adults experiencing mental health issues
  • Minimum of 3 years of supervisory experience
  • Possess a valid unrestricted driver license and be insurable by FSL
  • Possess CPR/First Aid certification or ability to obtain within 60 days of position entry
  • Possess Valid Food Handlers Card or ability to obtain within 60 days of position entry
  • Meet all state and federal immunization and communicable disease clearance requirements with or without an accommodation or exemption
  • Clear fingerprint background screening through the Department of Public Safety and comprehensive background clearance to include a physical, drug screen and other
  • Must be at least 21 years of age

Physical Demands

  • Ability to frequently stand, see, hear, speak, and be physically flexible; occasionally sits walk, stoop, lift up to 50 lbs., carry, run and drive; and at times may bend, climb, squat, kneel, push/pull
