Asbury Place - Maryville proudly provides high quality, compassionate services in skilled nursing, Assisted Living, Residential Living, post-acute rehabilitation, and memory support care for residents of Blount County and surrounding areas. As a not-for-profit organization committed to meeting the community’s needs, Asbury Place - Maryville is an important link in the Blount County health care delivery system. Asbury Place - Maryville is part of the Asbury Communities system, a not-for-profit aging services organization providing life-enhancing services to older adults in Maryland, Pennsylvania, and Tennessee.
Responsibilities:
- Tracks/trends referrals/admits/discharges
- Coordinates and collaborates with referring Case Managers and Discharge Planners. Cultivates and maintains relationships with referral sources
- Conducts outreach with hospitals and referral sources on a routine basis/as able
- Reviews/assesses clinical records/discharge summary prior to admission to assess appropriateness for placement
- Partners with DON/ADON/Nurse Manager to review appropriateness for placement
- Fully completes admission forms on all new admissions or readmissions
- Obtains insurance authorizations for new admission(s)
- Identifies opportunities for new partnerships
Requirements:
- Nursing or Case Management experience required
- Prefer previous Long-Term Care experience preferred
- High School Graduate or equivalent required
- AA or better preferred
