Director of Nursing - Care Management - Physicians Group of Texas Job at Kelsey-Seybold Clinic
Responsibilities
The Director, of Disease Management for the Physicians Group of Texas (PGT), will report to the CMO and has primary responsibility for care management within the entity and contracted partners. This individual will own the build-out of PGT’s care management strategies and team. He or she works in conjunction with the clinically integrated physicians, staff of participating hospitals and post-acute partners, and PGT senior management to coordinate care/disease management services for the entity. This position is a key stakeholder in ensuring the success of PGT’s clinically integrated network and goals.
The Director, Disease Management is a remote position with up to 25% travel required. All candidates must be licensed in the state of Texas. Preference for candidates that live in the state of Texas.
EDUCATION REQUIREMENTS & EXPERIENCE REQUIREMENTS
Bachelor's Degree or equivalent work experience
- 8-10 years of management in the healthcare industry that includes 5 years of direct experience in an ACO, CIN, IPA, or health plan in disease management and/or complex care management
- Strong working knowledge of industry regulations including Medicare and Commercial HMOs and those covering ACO, CIN, and IPAs in related organizations
- Background providing clinical expertise and business direction in support of medical care management programs through participation in clinical team activities, policy development, program performance monitoring
Preferred Requirements
TX RN Licensure, Bachelor or Masters Degree in Nursing, LCSW or Behavioral Health Credential
- Experience in team building, workforce planning, and expense management
- Experience in entrepreneurial environments
- Prior oversight managing clinical initiatives across geographically dispersed stakeholders and physician partners
- History of implementing risk adjustment and coding initiatives
- Experience managing and incorporating SDOH criteria into core services
Job Type: Full-time
Pay: $105,000.00 - $136,000.00 per year
