Aging and Long Term Support Administration seeks a Social and Health Program Consultant 4 assigned the responsibility for statewide development of partnerships and resources to increase community access for individuals with complex needs across the State's three Home and Community Services (HCS) regions. In this role, will collaborate with data technology, the Systems Change Specialist, and RCL Enrollment Specialists team to create, analyze, review and amend data collection and reporting methods to strengthen data integrity regarding transitions from nursing facilities and hospitals to the community which will be used to inform policy and procedure recommendations.

This is a primarily telework opportunity, and there may be some flexibility in terms of primary duty station assignment to other HCS offices. This job does involve state and local travel.

Some of what you'll do:

  • Create and utilize existing reports and outreach efforts to maximize RCL Participation data integrity
  • Utilize data, facilitate performance improvement with a focus in services provided to improve transitions from an institutional setting to a community setting
  • Provide evaluation, analysis, development, planning, and implementation of systems impacting the quality of care and services delivered to participants before, during and after transition to a community setting
  • Monitor quality assurance and quality improvement compliance with state and federal standards for individuals moving from institutional settings and take necessary actions as required
  • Assist with implementing new RCL services to include policies, procedures and integration into Medicaid state plans and waiver programs
  • Build knowledge systems, use change management concepts, work processes and process improvement methodologies, tools, and techniques to promote the development and enrichment of an organization-wide improvement culture
  • Identify and develop tools to objectively measure and track data related to client resource use and outcomes

Who should apply?
Applicants who possess a bachelor's degree in social work, health or social science, public administration or related field AND 5 years of experience with coordinating community services; or a master's degree and additional experience with long term care systems.
