JOB TITLE: Engagement/Outreach Specialist

PROGRAM: Healthy Families Niagara/Orleans

STATUS: Full Time, (35 Hours Per Week)

FLSA DESIGNATION: Non Exempt

REPORTS TO: Supervisor/Program Director of Healthy Families

DEGREE: AAS Human Services preferred; High School Diploma or GED required

EXPERIENCE: Experience working in HFNY Program

PAY RANGE: $18-$19 Per Hour


POSITION OVERVIEW:

The Engagement Outreach/ Coordinator supports HFN/O program, program goals and the team members while supporting capacity building. Outreach / Engagement Coordinator has an understanding HFA Best Practice Standards and the knowledge of PCS mission of services.


QUALIFICATIONS/EXPERIENCE:

  • Associates Degree preferred, High School diploma or GED required
  • Valid NYS Driver License and use of personal vehicle during work hours
  • Experience working in Healthy Families with children and families
  • Knowledge of normal child growth and development required
  • Experience with home visiting and knowledge of community resources preferred
  • Ability to use Microsoft applications (i.e. Word, Excel, PowerPoint, Outlook, etc.), Internet/Intranet, web-based databases. Understanding of the use of email in the workplace
  • Experience with HFNY MIS system


JOB RESPONSIBILITIES:

Outreach/Engagement Specialist:

  • Develop and create engagement strategies for all Creative Outreach (CO), Temporarily Reassigned (TR) cases
  • Review CO and TR activities in the Management Information System (MIS)
  • Planning and executing outreach events
  • Develop and maintain relationships with community partners
  • Identifying resources and community services that can benefit families in the program
  • Present to all HFN/O stakeholder about Healthy Families Niagara/ Orleans and updates
  • Conduct FROG assessments when needed
  • Support on boarding of new team members (schedule shadowing, training binder, etc.)
  • Hold small caseload as assigned when team members resign from position or on medical leave
  • Provide informational support groups to families who have not enrolled or accept Home visiting
  • Collect data to share with Program coordination to assess trends, new strategies and trainings if needed
  • Maintain tracking form of all events, presentations, and referral sources with contact person
  • Support training of team members on presentation and community outreach


GENERAL:

  • Assists with obtaining donations ordering and tracking supplies and any other projects assigned by your Supervisor
  • Attends and participates in all trainings and continues education provided by HFNY or Agency
  • Meets on a weekly basis for a minimum of two (2) hours per week with Supervisor at a regularly scheduled time.
  • Maintains confidentiality of participants and Agency


PHYSICAL/ENVIRONMENTAL DEMANDS:

  • Daily local travel; occasional distant travel
  • Occasional lifting (15 pounds or less)
  • Occasional standing/sitting
  • Moderate stress
  • PC/computer work
  • Live in or near target area
  • Available to work during non-typical work hours

  • Demonstrates knowledge of the program, mission, and services of PCS
  • Must have ability to relate to people with respect to their individuality and cultural diversity
  • Must be non-judgmental and believe in nurturing non-violent discipline of children
  • Attends and participates in appropriate Agency staff trainings and meetings
  • Advocates for cultural diversity/sensitivity as appropriate
  • Experience and knowledge on the use of computers, cell phones and a broad understanding of using the Internet and e-mail in the workplace
